Thomas Jefferson Univ Meds History Pharmacy Tech in Abington, Pennsylvania

Reference #: 9301870 005202 The Medication History Pharmacy Tech reports to the Assistant Director of Pharmacy and is responsible for accurately collecting and entering current patient medications into the hospitalrsquo;s information system for utilization by the physicians and providers responsible for admitting and discharging the patient from the hospital. The primary function includes patient interviews and review of internal and external systems with relevant information. The Medication History Pharmacy Tech is responsible for relaying any outstanding transition of care issues to the lead pharmacist and/or physician. The Medication History Pharmacy Tech will also perform limited medication management and/or distribution activities under the supervision of a pharmacist. The Medication History Pharmacy Tech will perform assigned responsibilities according to department policies, procedures, standards and regulations within stated time frames.

Experience: 1+ years of Pharmacy Technician experience. Prior experience with medication history/reconciliation preferred. Retail pharmacy experience preferred. Education amp; Training: HS Diploma or equivalent required; Licensed Practical Nurse or Bachelorrsquo;s degree preferred. License, Certification amp; Registration: Certified Pharmacy Technician (CPhT); if LPN CPhT required within 1 year of hire. Other Requirements: Excellent written and verbal skills; Proficient with basic PC skills; Good knowledge of medications, strengths available, indications. Soft skills with regards to patient/family interactions Covid Vaccination is a requirement for employment at Jefferson for employees working at Jeffersonrsquo;s clinical entities or at the University.
