Registered Nurse (RN) Hospice

Hospice helps patients and their families prepare physically, emotionally and spiritually for the end of life. Our focus is on providing comfort, not cure; on family, not just the patient; and on quality of life, not duration. Cleveland Clinic's hospice team includes physicians certified in hospice and palliative medicine, nurses, social workers, chaplains, home health aides and volunteers.

A caregiver in this position works day or night shifts from 7:00AM - 7:30PM or 7:00PM - 7:30AM. PRN caregivers are required to work 48 hours every six weeks, including at least one weekend shift.

A caregiver who excels in this role will:

• Provide supervision, assessment, planning and direct patient care to hospice patients.

• Perform accurate and complete admissions assessments and re-assessments.

• Assess patients for safety risks, abuse and neglect.

• Assess, document and manage patient's pain.

• Prioritize and document patient problems.

• Administer medications and treatments safely and accurately with a focus on patient comfort.

• Coordinate care for managing a caseload of patients.

• Document changes in patient condition and effectiveness and timeliness of care/interventions.

• Evaluate, update and revise plans of care.

• Address educational needs of patients, caregivers and family while considering cultural issues and other special challenges.

• Provide teaching and educational material to patients and families concerning diagnosis, diagnostic tests and procedures, safe and proper use of equipment, reason for and safe use of medication food/drug interactions, injury prevention, self-care and nutritional needs.

Minimum qualifications for the ideal future caregiver include:

• Graduate from an accredited school of nursing.

• Current licensure as a Registered Nurse (RN) in the State of Ohio.

• Basic Life Support (BLS) certification from the American Heart Association (AHA).

• One year of current clinical experience as an RN.

• One year of experience in an acute medical-surgical or critical care setting.

• Field Staff: valid Ohio driver's license.

• Preferred qualifications for the ideal future caregiver include:

• Bachelor of science in nursing (BSN).

• Hospice and Palliative Care Nurse certification.

Physical Requirements:

• Requires full range of body motion including handling and lifting packages, manual and finger dexterity, and eye-hand coordination.

• Requires standing and walking for extensive periods of time.

• Heavy work - Exerting 50 to 100 pounds of force occasionally, and/or 25 to 50 pounds of force frequently, and/or 10 to 20 pounds of force constantly to move objects. Physical Demand requirements are in excess of those for Medium Work.
