Corus Health Nurse Practitioner (NP) - Valencia County in Albuquerque, New Mexico

As a full-service medical practice provider, Corus Medical is seeking a qualified, Nurse Practitioner who will be responsible for caring for patients, maintaining accurate and current patient records, and administering initial patient assessments and follow-up appointments as required. Our NP will collaborate with our team of nurses and physicians to deliver premium care for our valued patients to assist with successful transitions through a variety of settings including hospitals, transitional care facilities, long-term care, assisted livings, and private homes.
Responsibilities & Duties:
Performing initial patient screenings, patient history, and physical examinations.
Updating patient records and cross-checking records for accuracy at each appointment.
Delivering patient-centered care according to clinical status, as well as patients’ personal goals and preferences.
Performing hospice face to face assessments as required by Medicare & insurance to determine if patients qualify for services.
Serving as a resource to our patients for ongoing care information, counselling and providing guidance for patients and their families.
Assisting patients and families with appropriate logistical arrangements.
Ability to work well and collaborate as an integral part of a team of nurses and physicians.
Requirements
Qualifications & Requirements:
Master’s Degree in Nursing
2+ years of applicable nursing and work experience, at least 1 year of home health experience preferred
Valid Nurse Practitioner Licensure in the state of New Mexico
Driver’s License and reliable, insured transportation
Skilled in nursing practice, and able to cope with family emotional stress and tolerant of individual lifestyles
Strong written and interpersonal communication skills
Valid driver’s license and reliable, insured transportation
