    Minimum Education and/or Professional Qualifications/Skills:

  • Bachelor’s degree in Human Services or related field + 2 years of relevant experience*

  • High School Diploma + 4 years of relevant experience* *Relevant experience includes care coordination, community-based client services, community health work, or roles in healthcare, public health, non-profits, or similar fields.

Full Job Description Title: Health Related Social Needs Navigator Department: Health Related Social Needs Services Title of Immediate Supervisor: Health Related Social Needs Services Supervisor POSITION SUMMARY: The HRSN Navigator will provide screenings and navigation services for anyone who meets the eligibility criteria within the WNY or Finger Lakes regions (WNYICC/FLIPA). The Navigator will take a holistic approach to coordinating a range of services to best meet the individual’s needs. The Navigator will utilize a strength-based, trauma informed, and person centered approach for screening/navigation. Screening and navigation are provided in home and community settings. RESPONSIBILITIES/DUTIES:  Navigator will provide empathetic and culturally competent screening for social determinants for health to community members telephonically, virtually and/or in-person.  Process screenings and referral intake daily including calls with potential program participants, documentation, and insurance/eligibility verification  Navigator will receive, prioritize, monitor and triage screenings and referrals that require services connections across community-based and organizations  Navigator will document progress and action taken with each referral and every interaction in the overseeing body (WNYICC/FLIPA) IT platforms as required.  Navigator will conduct telephonic, virtual or in home assessments with community members  Maintain effective communication with community members and/or referring organizations to ensure acceptance and resolution of referrals by the receiving organizations or to redirect the referral to another provider if necessary  Navigator will refer eligible youth from 0-21 to health homes and other Medicaid funded programs within agency  Attend meetings/workgroups with the overseeing social care network  Documentation will be done within the IT platform and transportation platforms within WNYICC/FLIPA  Documentation will be completed within the necessary timeframes created by the overseeing SCN  Maintain 120hrs of billable services per month  Provide support to members who have questions or concerns regarding needs and services Additional Responsibilities: As assigned by immediate supervisor Agency Standards/Norms:  Participate in regular supervision (per New Directions policy and procedure).  Communicate schedule to supervisor on a weekly basis. Update changes as they occur.  Attend all mandatory in-service trainings as per NDYFS policy and requirements.  Participate in continuing education as needed to maintain a level of expertise in the field.  Adhere to agency and department policies and procedures.  Utilize normative culture.  Adhere to the Code of Ethics prescribed by the National Association of Social Workers.  Perform all other duties as assigned by immediate supervisor.
