Schedule: PRN (8-10+hours per week)

Coverage Area: Westford to Peabody to Salisbury

Summary of Duties:

  • Alignment with Vision: Ensure all activities align with Elara Caring’s leadership vision and mission.

  • Patient Assessment: Evaluate patients' psychosocial, financial, and environmental needs through various documentation methods.

  • Social Work Visits: Conduct initial and follow-up social work evaluations, adjusting the Plan of Care as needed.

  • Communication: Report significant findings, unsafe conditions, and visit outcomes to the Supervisor and involved care personnel.

  • Patient Safety: Implement and revise safety plans using available resources, preventing avoidable hospitalizations and ER visits.

  • Plan of Care: Develop, verify, and update the Plan of Care, ensuring continuity and quality of care according to physician orders.

  • Counseling and Support: Provide counseling and instruct patients and families on following the Plan of Care.

  • Compliance Monitoring: Ensure adherence to third-party payor requirements, maintaining open communication with clinical staff.

  • Collaborative Care: Participate in patient care conferences and provide supportive communication to all stakeholders.

  • Physical Demands: Handle physically demanding tasks including patient lifting, equipment handling, and exposure to various health risks, while maintaining safety and confidentiality standards.

What We're Looking For:

  • Master’s degree or Doctoral degree in social work from a social work from a school of social work accredited by the Council of Social Work Education

  • Current state license as a Clinical Social Worker > either LiCSW or LCSW

  • 1 year of social work experience in a healthcare setting

  • Social work skills as defined as generally social work standards

  • Valid driver’s license and insurance and reliable transportation to perform job tasks
