Intermountain Health Bereavement Coordinator in Annapolis, Maryland
Job Description:
Coordinates bereavement program and the delivery of related services for a facility or service line, including coordination with community grief support groups and other community outreach events or programs. Provides direct counseling and support to patients and families as part of the bereavement program services.

Shift : As needed, flexible schedule with evening and weekend potential.
This position will represent the Angel Watch program to provide bereavement support primarily for families residing in Utah County. In-home visits are expected in this role.
Job Essentials
Coordinates bereavement programs and events for a facility or service line.
Evaluates bereavement activities and services. Educates and trains program staff and volunteers, and community members in efforts to effectively address grief and loss in a timely and meaningful manner.
Provides counseling and psychosocial assessment for grieving and soon to be bereaved families, and refers families to appropriate community agencies based on consultation and/or coordination with other related services.
Coordinates site or service line programs such as maintaining the bereavement database, arranging for mailings or other communication, and event planning.
With input from the applicable clinical team and family members, may develop treatment plan for appropriate intervention for patient/families. May coordinate with other team members on families? progress and changes in the plan of care, including participation in regular conferences with team members and supervisors. May prepare clinical and progress reports and record-keeping.
Collaborates with management to better educate the community at large about the bereavement process and to raise the level of awareness of the communal obligation to those bereaved. Develops relationships with community bereavement resources.
Communicates effectively and tactfully with clients, recognizing their age, cultural diversity, needs, abilities and physical condition.
Stays current with bereavement literature and best practices. Performs research and attends trainings as appropriate.
Assesses and implements local bereavement quality improvement initiatives.
Minimum Qualifications
Two years of experience in bereavement counseling in an RN, Social Worker, chaplaincy or clinic grief counseling role.
Current clinical licensure as an RN, Social Worker or certification from a nationally recognized chaplaincy or grief counseling program
Preferred Qualifications
5 years previous bereavement counseling experience in a hospital, hospice, or palliative care environment.
For pediatric facilities or service lines, 2 years minimum prior pediatric bereavement coordination experience.
Physical Requirements:
Interact with others requiring the employee to communicate information.
Operate computers and other office equipment requiring the ability to move fingers and hands.
See and read computer monitors and documents.
Remain sitting or standing for long periods of time to perform work on a computer, telephone, or other equipment.
