Columbia Memorial Hospital Clinical Social Worker - CSWA/LCSW in Astoria, Oregon

The Clinical Social Work Associate (CSWA)/Licensed Clinical Social Worker (LCSW) works as a clinical member of an interdisciplinary care team providing brief assessment, intervention, and onsite consultation regarding psychiatric/psychosocial concerns and health behavior change. The CSWA/LCSW works across the continuum of care to improve population health outcomes by ensuring patients of all ages have access to behavioral health expertise and psychosocial supports in all points of care. In the outpatient setting, the CSWA/LCSW is integrated into primary care and specialty clinics to provide brief, solution focused services. All CSWAs/LCSWs are members of the Care Management department at Columbia Memorial Hospital (CMH), which allows for more collaboration and coordination across the continuum as well as for clinical supervision and cross-coverage as needed.
Job Requirements Knowledge of/Skill/Ability to: Ability to provide strengths-based, biopsychosocial assessment and evidence-based interventions to patients of all ages with behavioral health concerns, substance abuse and health compromising behaviors. Ability to provide psychoeducation to patients and their support systems. Ability to provide psychotherapy services and complete documentation required for billing. Ability to quickly build rapport for brief as well as ongoing clinical assessments of behavioral health, resource and care coordination needs. Ability to function as an integrated member of the interdisciplinary medical team with strong communication skills. Ability to work in a brief, solution focused model as well as a long-term therapeutic model of care, while connecting patients to community resources and higher levels of behavioral health care, as indicated. Ability to maintain the code of ethics and standards of Social Work, in accordance with principles set forth by the National Association of Social Work (NASW), CMH, and agency policies and procedures. Excellent interpersonal skills and ability to work independently, as well as collaboratively with medical staff, patients, and other clinicians.
License/Certification
- If CSWA, Oregon Clinical Social Work Associate licensure required, or ability to obtain within 120 days of hire. Ability to obtain LCSW within 3 years of start of full time employment; for employment status outside of full time, a mutually agreed upon individualized timeline to obtain LCSW will be determined. - If LCSW, Oregon Licensed Clinical Social Work required, or ability to transfer licensure from another state within 60 days of hire. Ability to maintain credentialing and appropriate documentation for insurance companies if needed.
Education and/or Experience Masters in Social Work from CSWE accredited school; active CSWA/LCSW license in the state of Oregon. Minimum of one year post-Masters experience in medical social work preferred. Competency in providing cognitive and behavioral assessments and interventions to individuals, families, couples, and groups with behavioral health and/or substance use complications in a health care setting.
Physical Demands and Work Environment Conditions / ADA Compliance Involves standing, walking, stooping, and occasional carrying/lifting of lightweight materials. May be required to sit or stand for extended periods of time. Requires visual acuity and depth perception to recognize words/numbers Requires hand/finger dexterity and hand/eye coordination to use a computer keyboard and common office equipment.
