Job Overview

The Health Navigator serves as an intermediary between the health care system, social services and the community to facilitate access to services in support of families and assist in the obtainingof care and resources needed to maintain their health. Navigators reduce barriers that keep patients from getting timely treatment by identifying patient needs and directing them to sources of clinical, financial, or social support. The Health Navigator may be responsible for assessment, planning and referrals to appropriate resources. Role will mostly interact with families via phone or virtual modalities. Work primarily requires being on a computer or phone and includes documentation via Epic EMR, email communications and phone communications. This position will become part of the team of Asthma Health Navigators, which will work collaboratively to meet the needs of patients and families. This navigation role is tailored to asthma populations and will work with two different teams in a 50/50 time split. One of the teams is part of the Asthma Inpatient Navigation and the other team will be part of virtual Asthma Navigation in the outpatient setting.

Qualifications

EDUCATION - High School Diploma or equivalent. EXPERIENCE - Minimum 3 years of experience assisting patients and families or other related healthcare/community experience that includes working with immigrant populations that communicate in languages other than English. Spanish speaker preferred. Knowledge of State social service agencies and community resources. Knowledge with various electronic documentation and reporting programs preferred. EQUIVALENCY - An Associate degree, in a related field, may substitute for the experience requirements on a year for year basis.

Assists families in community and home settings as determined by established algorithm to remove barriers related to access services. Refers families to clinical, social, and financial supports using an established algorithm to assess patient and family needs. Works with staff and providers to create a care plan and communicates with them regarding the status of referrals and outreach efforts. Works with individuals from diverse backgrounds in reducing cultural and socio-economic barriers between clients and institutions.Builds family capacity to address health and social issues through education, care
