Responsibilities

As a Licensed Case Manager, (LMSW or LCSW) , The CM will be in the physician's office and meet with the member before or after their prenatal or postpartum visit and complete the health risk assessment, identify any barriers and enroll the member in the USDA Food Farmacy and Maternity Social Initiatives program.

You will develop initiatives to improve member outcomes and expand reach of services. Coordinate comprehensive plan of care for patients.

  • Oversee primary needs of patient, including assessment of client and family/caregivers, and develop comprehensive plan of care.

  • Provide primary home care/consultation to complex client and family needs within area of expertise and in collaboration with physician.

  • Monitor patient care through assessments, evaluations and/or patient records.

  • Advocate for resources and removal of barriers.

  • Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan.

Requirements

Licensure / Certification / Registration:

  • Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Licensure required relevant to state in which work is performed.

  • Counselor preferred. Licensure required relevant to state in which work is performed.

  • Social Worker preferred. Licensure required relevant to state in which work is performed.

  • BLS Provider. American Heart Association or American Red Cross preferred.

Education:

  • High School diploma equivalency with 2 years of cumulative experience OR Associate's degree/Technical degree OR 4 years of applicable cumulative job specific experience required.

Additional Preferences

  • Bachelor’s or master’s degree in social work

  • 3+ years of clinical social work experience

  • Valid and current LCSW or LMSW license

  • Ability to make psychosocial assessments and develop and implement care plans

  • Ability to provide after-hours service as needed

  • Bilingual preferred

  • Maternal Health experience

  • Knowledge of local community resources

  • Health plan experience - Medicaid STAR is a plus.
