Type: Four month contract, opportunity to get an extension

Hours: Monday to Friday 8am to 5PM

Pay: $32.85 to $ 34.85/hr

Location: Austin, TX - Role does require some travel around the Austin area. (Mileage is reimbursed)

Primary Responsibilities:

· Generating and distributing reports using secure file exchange

· Educating and training providers and staff on quality measures and best practices

· Goal setting and accountability

· Support in telephonic outreaches to patients as needed.

Qualifications:

• High School Diploma/GED or Equivalent

• Current unrestricted Vocational Nurse License in Texas

• 3-5 years of LVN exp

• Bilingual (English/Spanish) preferred

• Immunizations/vaccinations required: MMR, TB Screening, Varicella, Hep B, Flu shot, Tdap, Covid 19 vaccination
