Job/Position DescriptionProgram Specialist VUnder the general direction of the Manager for the Diabetes and Cardiovascular Disease (DCVD) Branch, in the Health Promotion and Chronic Disease Prevention (HPCDP) Section within the Community Health Improvement Division, performs highly advanced public health work and serves as the learning collaborative coordinator for the Heart Disease and Stroke Program (HDSP). Work includes, but is not limited to, coordination of two learning collaboratives, strategic planning, program and grant implementation, stakeholder engagement, and contract oversight. Assists with establishing program goals and objectives, development of activities, priorities and the standards for achieving them. Establishes, maintains and supports collaboration between a diverse group of key partners including public health agencies, academic institutions, community organizations, health systems, and other subject matter experts and stakeholders. Aligns learning collaborative objectives with federal grant strategies, the CVD State Plan, and the Texas Council on Cardiovascular Disease and Stroke (TCCVDS). Develops and monitors internal and external partnerships to expand collaboration. Assists in the development of grant applications and funding proposals and identifies and monitors progress on grant activities. Develops contractor scopes of work and assists with monitoring activities, reviewing quality assurance standards, and providing technical assistance. Works collaboratively with other programs in the HPCDP Section to identify best practices and areas for improvement for chronic disease-related community and clinical services. Works under minimal supervision with considerable latitude for the use of initiative and independent judgment.Essential Job FunctionsAttends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.(30%) Serves as the HDSP liason for internal and external workgroups or committees that address coordination of chronic disease prevention and treatment efforts. Develops and mantains internal and external partnerships to expand collaboration and disseminate best practices and lessons learned related to cardiovascular disease and stroke interventions. Identifies new opportunities for partnership development and engagement to build learning collaborative membership. Establishes, supports and maintains collaboration among learning collaborative members. Ensures learning collaborative membership is composed of a diverse group of partners and stakeholders. Aligns learning collaborative activities, objectives and goals with federal grant strategies, CVD State Plan and the TCCVDS. Works collaboratively with other programs in the HPCDP section and the Office of Public Health Policy to identify best practices and areas for improvement in community and clinical services related to chronic disease. (30%) Assists in the development of grant applications, activities, budgets, work plans and progress reports. Develops contracts, including writing scopes of work, budgets and deliverables. Monitors contracts, and provides technical assistance to contractors on implementation of activities and evidence-based strategies. Collaborates with the evaluation team to track performance measures, evaluate contracted activities and deliverables, and identify opportunities for improvement.
