Position Description

Penn Medicine at Home is seeking a per diem Nurse Practitioner to join their growing team! The Nurse Practitioner will be cross-trained to provide Palliative Care and Primary Care services to adult patients in the home setting seeing a combination of palliative care consults in the home, as well as primary and transitional care. Shift opportunities exist Monday through Friday from 8:30am to 5:00pm across Philadelphia, Delaware County, Montgomery County, Chester County, and Bucks County.

Minimum Requirements

Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania, and possess a MSN or DNP and certification as a Adult Gerontology Nurse Practitioner or Family Nurse Practitioner. The ideal candidate will have related Nurse Practitioner and home care experience.
