Under the direction of the Associate Director of Community Supports, this position ensures person-centered-services are provided. This position will ensure all service delivery plans under the program’s auspices, are written in a cohesive manner, ensuring services are aligned with one another to reflect the agency’s person-centered philosophy and approach to service planning and delivery. This position will ensure communication and information distribution within each person’s circle of support. This includes the person, their natural supports, Care Coordinators/Care Managers, agency services, and external services. This position will be a “Super User” of the Agency Management Platform (Therap) and will support all departments with the system. The Planning and Intake Specialist I will also facilitate intake for the traditional services offered by the organization. Each Specialist I will be the central point of contact to facilitate referral for a service from the Care Coordinator or other party.

JOB FUNCTIONS:

Regulatory and Corporate Compliance

  • Ensure conformance with applicable regulations and policies.

  • Ensure records, files and reports are kept current and meet regulatory requirements (both in the program of service and the Agency Management Platform).

  • Develop and implement improvements and corrective actions based on survey results.

  • Ensure associates understand and accurately implement plans of service.

  • Ensure adherence to incident management process.

  • Review, monitor, and take corrective action to ensure systems and processes are effective.

  • Ensure Chain of Command is followed when necessary and instructions are followed as stated in policy and procedures.

    Financial Management

  • Adhere to the program budget.

  • Ensure accurate management and documentation of individual and program funds.

  • Ensure waiver billing is completed accurately.

  • Investigate and ensure corrective action is taken for waiver billing concerns.

  • Complete audits of monthly summary reports and follows up with appropriate department for any instances of incomplete or inaccurate documentation or missed assignments.

    Person Centered Services

  • Advocate with the person in the development of his/her Life Plan.

  • Ensure the person’s Life Plan, assessments, training, and objectives are person directed and reflect the person’s quality of life choices and support systems.

  • Facilitate the review and update of Person-Centered Frameworks.

  • Oversee the development and authors the person’s Staff Action Plan; utilizing Personal Outcome Measures, valued outcomes, and Provider Assigned Goals from the Life Plan, as well as input from the person and his/her circle of support.

  • Observe the implementation of the Staff Action Plan in program and community settings to ensure the Staff Action Plan is implemented and documented, as intended and required.

  • Regularly attend program planning team meetings to ensure communication with program/management staff. Assist programs with understanding the Life Plan, Staff Action Plan, and safeguards.

  • Assist the Behavior Intervention Specialist with gathering and distributing information with the person and the IDT.

  • Provide training, feedback, reinforcement, and corrective coaching to associates’ implementation of the Staff Action Plan.

  • Respect the Person’s right to privacy and confidentiality by appropriate use of written and verbal communication.

  • Be an active member of each person’s Interdisciplinary Team (IDT).

  • Work collaboratively with others to ensure the person’s health and well-being.

  • Be a Personal Outcome Assessment Interviewer and complete assigned assessments within designated time frame.

  • Actively participate in the development of and achievement of the person’s identified personal outcomes.

  • Work closely with service providers and Care Coordinators/Care Managers to ensure effective communication and that Staff Action Plans are coordinated and consistent.

  • Facilitate person centered planning in the intake process.

  • Support families to navigate the OPWDD system.

    Program/Service Management

  • Maintain an Agency wide procedure for the admission of individuals seeking services, ensuring the efficiency and effectiveness of these procedures.

  • Maintain a quality system for Central Intake to include wait lists for services and causes for delays in service receipt.

  • Provide data to the Associate Director of Planning and Intake regarding the Centralized Intake systems and processes.

  • Present all available Agency services and assist prospective persons to be served and his/her family during the entire Intake process. F acilitate tours of Agency programs as requested by the prospective person to be served and his/her family.

  • Serve as the primary liaison throughout the Intake process between the referral source, prospective person/family to be served, ACCESS team of Region I DDRO and agency programs or services.

  • Complete all Intake forms as prescribed by the Agency’s Central Intake Policy and ensure distribution of completed Intake packets for requested services.

  • Provide persons and families with education to better understand the intake process and service options and choices.

  • Act as an advocate for prospective persons to be served in obtaining services requested within the Agency.

  • Maintain reasonable flexibility in schedule to accommodate people served.

  • Participate and observe the provision of services and reviews service documentation to ensure the service plan is being implemented as intended.

  • Review and approve monthly summaries to ensure supporting documentation meets regulatory compliance and supports the person’s valued outcomes.

  • Ensure each person’s information is kept up to date and assessments are completed as required in the Agency Management Platforms, such as Therap, and OPWDD Platforms, such as CHOICES.

  • Complete and ensure the person’s DDP2 is current and accurate for each service. Verifies that all DDPs are in alignment with abilities skills, needs, and safeguards. Enters into OPWDD CHOICES platform.

  • Review the CAS Assessment to ensure the information is included in the person’s applicable service plans.

  • Facilitate the review and update for the Planning team’s Determination of Capacity to Consent to Restrictive Interventions.

  • Ensure service plan reviews are completed and distributed according to regulation.

  • Assist in the identification and development of new services and supports that provide quality of life choices.

  • Provide training and support in use of Therap.

  • Work in collaboration with Program Managers through the referral process and startup of new services for each person.

  • Distribution of all agency Staff Action Plans, as well as Safeguards, Individual Plan of Protection (IPOP), Plans of Nursing Services (PONS), Behavior Support Plans (BSP), and Person Centered Frameworks.

    Physical Plant and Safety

  • Ensure the site is a safe environment and immediately address areas of concern.

  • Practice standard precautions and other appropriate infection control practices.

  • Maintain knowledge of fire evacuation plan and ensure plan is followed.

  • Report and document possible safety hazards

    Contacts with Others

  • Develop and maintain positive relationships with internal and external stakeholders and demonstrates high diplomacy. This includes the person, their family, local businesses, Care Coordinators/Care Managers, and external service providers.

  • Maintain effective working relationships with all service providers, community resources, and natural supports.

  • Ensure that internal and external written and verbal communications regarding the agency are positive and reflect our mission, vision, and values, and respect the person’s right to privacy and confidentially.

  • Ensure concerns are addressed in a timely and professional manner.

  • Demonstrate respect for diversity and inclusion.

  • Meet with individuals, family members and members of a person’s circle of support on a routine basis through intake meetings and person centered planning to develop an effective plan for service for the individual.

  • May be required to use private vehicle to go to meetings and possibly to transport the people we support.

  • Identify opportunities to use Therap fully and ensures these opportunities are maximized.

  • Work with the Therap team to identify and develop new services and programs to utilize Therap.

  • Assist program associates with using Therap .

  • Provide and document for services provided in applicable programs, such as but not limited to Community PreVoc (CPV), Supported Employment (SEMP), and Pathway To Employment (PTE).

  • Is a Therap and CHOICES “Superuser” for the agency.

  • The person in this position has basic Microsoft Office skills

  • The person in this position has basic Microsoft Excel skills, including the ability to enter and sort data and produce graphs.

  • The person in this position demonstrates knowledge of introductory accounting and computer skills.

  • The person in this position has good written and verbal communication skills and possesses counseling and crisis management skills as well as leadership.

  • The person in this position must maintain a working telephone.

    QUALIFICATIONS

    • Associate degree in Human Services with one year experience with people with developmental disabilities is preferred.
  • OR High School Diploma with three years' experience working with people with developmental or intellectual disabilities is required.

  • CPR/First Aid/AED and SCIP Certification within the first six months of hire.

  • Innovations, Personal Outcome Measures, and Person Centered Thinking trainings within one year of hire.

  • Alignment with agency’s mission, vision and code of ethics.

  • Demonstrate respectfulness, professionalism, effective teamwork and empathy such as active listening.

    ADDITIONAL

    QUALIFICATIONS/REQUIREMENTS:

  • Where position requires driving:

  • Valid driver’s license for the state in which the person resides

  • Acceptable safe driving record

  • Where position requires driving individuals served by the Agency:

  • Motor vehicle with up-to-date inspection, that is registered and insured

  • Will be required to enroll in LENS (Licensed Event Notification System).

    Salary - $17.22/hour
