PC18-3. Job Description: Certified Caregiver The Certified Caregivers of Lifegiving Essentials LLC provides activities of daily living (ADL) and instrumental activities of daily living (IADL) service to clients who pay privately or are covered by insurance that requires a certified home care aide to provide the services. QUALIFICATIONS: 1. Must be at least 18 years of age 2. Prefer high school degree or GED 3. Must have at least one year of employment experience as a Basic Personal Care Assistant or in a related field, or else deemed an appropriate candidate willing to be trained in all necessary job aspects 4. Proof of current CPR training preferred, but will be supplied for the right candidate 5. Proof of successful completion a minimum of 40 hours of training and/or demonstrated competency in and knowledge of: a. Responsibilities and limitations communication techniques b. Client rights confidentiality of PHI c. Respect for person and property d. Observation for client changes to report e. Standard infection control f. Home safety precautions g. Basic body functions and variances h. Procedures for clean, safe environment i. Emergency response j. Physical, emotional, and mental client care k. Bed, sponge, tub, or shower bath l. Skin care, oral hygiene, toileting m. Basic nutrition, meal preparation n. Encouraging independent care o. Documentation techniques p. Supervisory support and duties q. Advance directives implications 6. Ability to communicate with clients and to remain calm under stress 7. Ability to read and write in English 8. A state Certified Home Care Aide or state Certified Nursing Assistant or pass the agency requirements for an Certified Caregiver may perform these duties 9. Ability to follow oral and written directions and complete work as assigned 10. Ability to maintain confidentiality about client services and personal information 11. Maintain current state driver's license and dependable insured automobile 12. Health status good and ability to lift, bend, squat the level and amount determined by each job assigned 13. Accurately document Certified Caregiver services provided 14. Present a neat appearance and positive work attitude RESPONSIBILITIES: 1. Demonstrates support for the agency mission statement to promote quality, efficient, comprehensive, and effective client services. 2. Participates in and supports the agency's performance improvement activities 3. Provides Certified Caregiver services to clients as identified by the agency in the plan of services per agency policies, procedures, and Certified Caregiver skill level. 4. Prepares and maintains accurate and timely client notes, documenting interventions, client pain score, outcomes, and client response per agency policy. 5. Assists with personal care activities (ADLs) and IADLs allowing client independence to tolerance as indicated in the service plan including: a. Personal care and hygiene b. Universal precautions c. Locomotion/ambulation/fall prevention oversight d. Nutritional support, meal preparation, feeding assistance e. Bed mobility and positioning, range of motion exercises f. Toileting g. Continence care h. Dressing and undressing i. Bathing j. Foot care (nails filed only) k. Skin care l. Medication reminder assistance m. Light housekeeping including dusting, vacuuming, linen changes n. Laundry of client's clothing and bedding o. Social and emotional support 6. Reports client condition changes to supervision. 7. Appropriately seeks supervisory assistance as necessary to effectively perform duties. 8. Maintains confidentiality of client, employee, and agency operations. 9. Other duties as assigned
