Performs intermediate skilled human support work supporting county members by improving quality outcomes to underserved populations and reducing health disparities, providing culturally appropriate health education, information, informal counseling, and outreach in community-based settings, including schools, shelters, community centers, bridging gaps between Social Services, Income Maintenance, and Public Health, ensuring and assisting people with access to the coverage and services they need, and related work as apparent or assigned.
