Qualifications:

  • A High School diploma or equivalent and course work in a Human Services certificate program, or an Associates degree in Human Services and 2 years of Residential Care experience, or 3 years of general experience and at least one year in management. A Bachelors degree in Human Services is preferred.
  • Windows software skills, both typing and navigating, and e-mail usage is required
  • Valid Driver\'s License

Requirements

The Program Coordinator is responsible for establishing connections with community employers and businesses to expand employment opportunities for Developmental Services (DS) clients. The Coordinator provides education to employers regarding working with clients, including job training and placement, job tailoring, problem solving, and ongoing supports and services for employers and clients. MAJOR RESPONSIBILITIES:

PROGRAM ADMINISTRATION AND SUPERVISION:Responsible for overseeing the Employment Connections Program. Provide leadership, training, and role modeling for staff. Ensures that all staff are familiar with division mission and philosophy and that this is being implemented across the program. Provides guidance and direction through the supervisory and evaluation process. Provides direct supervision and support to the Employment Specialist and DSPs. Contacts and schedules visits with employers for the purpose of explaining the Employment Connections Program, ensuring clients are being supported effectively and in line with employer expectations, and regular confirmation that DSPs are providing appropriate training and support.

Assists the Employment Specialist with establishing job placements and provides back-up job training and follow-up services for individuals as needed. Provides ongoing, follow-up and consultation to employers to help with individual adjustment, problem solve, discuss fadin supports and increased client independence, and ensures skill development of individual and suitable job matches. Ensures terms of vocational grants are met and followed. Participates in Developmental Services management team activities, projects, and meetings. Attends all trainings as assigned and ensures that staff are trained. Creates systems whereby all staff are appropriately trained on client support plans.

RECORD KEEPING:Reports agency data as requested by the division and the state and participates as a member of the state Supported Employment team attending monthly and/or quarterly meetings. Ensures all waiver services are scheduled and being received. Any waiver changes are reported promptly to the Division Director.

Completes accurate data collection reports for billing and management information systems in accordance with agency policy. Records supervisory and staff meeting minutes. Documents DSP progress towards Key Performance Indicators (KPI goals). Checks progress notes and monthly summaries for professional comprehensive writing. Completes progress repor
