The Developmental Disabilities Program procures and supports community-based services for people with developmental disabilities and/or their families. The DDP Targeted Case Manager is responsible for a comprehensive assessment and periodic reassessment of individual needs, the development of and periodic revision of the plan of care, referral and related activities to help the member obtain needed services, and monitoring and follow-up activities.
