Guthrie Medication History Technician - Emergency Services - Per Diem in Binghamton, New York

Position Summary:

The Medication History Technician performs all activities associated with medication reconciliation for Guthrie Robert Packer Hospital pharmacy and The Guthrie Clinic.

Education, License & Cert:

High school graduate or equivalent with a basic math and science background. Certified Pharmacy Technician certificate preferred

Experience:

A minimum of three year’s previous pharmacy experience in the health care environment is preferred.

Essential Functions: 1. Interviews patients and/or family members regarding medication and other related information. 2. Documents medication and other information accurately in the Electronic Medical Record to generate the best possible prior to admission medication list for provider reconciliation. 3. Collects appropriate medication and other information as deemed by the pharmacist in charge of the medication reconciliation program. 4. Utilizes resources within computer system such as previous dispense information and notes to ensure completeness of medication history and verify information. 5. Update medications prior to admission in electronic health record (HER) a. Create a unified, accurate medication history b. Provide an accurate medication list for providers and patients 6. Maintains appropriate queues in the Electronic Medical Record. 7. Reports to pharmacy for additional duties when time permits. 8. Understands the infection control procedures and wears appropriate garb when appropriate. Participates in performance improvement functions and data collection.

Other Duties: 1. Serves on committees as appropriate. 2. Participates in Quality Improvement process including: Identifying areas in which quality improvement process can be applied, collecting and organizing quality measurement data, and participating on Quality Improvement Teams. 3. Performs other duties as assigned. 4. Maintains appropriate confidentiality.

Pay Range: $16.00-$25.16/hr
