Humana RN Clinical Manager, Home Health in Boca Raton, Florida

The Case Manager is an RN responsible for maintaining the quality of patient care in accordance with the agencies objectives and policies, through planning, coordination, implementation and evaluating the home health services provided.

The RN Case Manager provides supervision of nurses and home health aides to ensure the quality of services and maintenance of standards. Reviews and investigates the clinical content, diagnosis, medical history, medications and other procedures of the record of patients under their supervision. Directs staff toward the appropriate use of community resources. Appraises performance at probation and annually with the Director of Nursing. Provides the teaching and counseling necessary to enhance the delivery of services and professional development of nursing professional and health aide staff. Identifies and evaluates nursing needs of the patient and appropriateness of home care during supervisory visits. Assists with the orientation of new professional staff to agency policies and procedures. Sees patients in the home setting as needed. Participates in audits, interdisciplinary team conferences and represents agency in the community as requested.

Reviews nurse's schedule for recertifications, discharges and home health aide supervisory visits. Reviews weekly schedule done by nurse for changes or updates.

Ensures HIPPA guidelines and procedures are maintained.

  • Minimum 2 years of experience. Maximum 5 years of experience

  • Graduate from an accredited School of Nursing.

  • Must be a licensed RN in the State of Florida

  • Minimum 2 or more years of experience in a home health agency

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.$76,800 - $105,800 per year
