Community Care Home Health Services (Copy) Home Health Aide (HHA) in Bronx, New York

FSLA: Non-Exempt Status: Full-time Pay Rate: $19.72 per hour, along with a $250 sign-on bonus

General Purpose:
To provide personal care and incidental/instrumental activities of daily living as assigned to patients/clients in the home setting as permitted by the policies and procedures of Community Care ("Company") and any other federal, state, local law or regulation.

Essential Functions: - Assist, perform, and train patient/client and family in activities of daily living such as bathing, grooming, toileting and elimination and adequate nutritional intake. - Promote safe environment and maintain unobstructed pathways while providing care. - Assist with ambulation, transfers, and/or range of motion exercises. - Assist with incidental/instrumental activities of daily living such as shopping, meal preparation, socialization activities, homemaking, maintaining a clean and safe physical environment, and reminding the patient/client to take their medication. - Utilize infection control measures such as universal precautions, hand washing, and personal protective equipment (PPE). - Recognize, document, report changes in patient/client condition and/or safety to the Supervisor/Manager. - Organize self to carry out visits/shifts and tasks. - Attend mandatory Company In-services and provide requested documents to keep employee file current. - Complete other assignments as requested and assigned. - May have access to personal health information necessary to fulfill the above duties and responsibilities. Access to use and ability to disclose PHI is further defined by each organization/department.

Minimum Education & Experience Requirements: HHA certification obtained by: - Successful completion of the HHA Training Program and approved by NYS Dept. of Health or NYS Education Dept.; or - Successful completion of approved HHA Competency Evaluation Program; or - Approved Personal care Aide Upgrade program.

Knowledge, Skills & Abilities Required: - Meets applicable health requirements to provide client services. - Able to effectively communicate with all levels of the workforce. - Able to hear, speak, write, and read in a manner understood by most people. - Able to read and interpret technical instructions related to providing services of the patient/client & use equipment required in providing service. - Able to prioritize multiple tasks to effectively deal w/multiple changes based on client needs & scheduling

Working Conditions & Physical Effort: - Work is normally performed in a resident or senior living facility - Work may involve frequent exposure to communicable diseases, bloodborne pathogens &/or other potentially infectious or hazardous materials that require following safety precautions & using work practices that require the use of personal protective equipment - Able to travel to assignments within geographic area served. - Medium physical activity that may require constant lifting, reposition, and transfer of clients & assist with standing & walking - Able to constantly stand, bend, stoop, twist, stretch, squat, kneel, walk, climb stairs, & reach freely. - Able to grasp with thumb in opposition to fingers of palm, i.e. manipulate pen, knobs, or objects. - Ability to visually & auditorily observe the client
