Rising Ground Hybrid Case Worker (Preventive Services - Fatherhood) in Bronx, New York

TheCase Manageris the primary contact for participating fathers in the program, contact each assigned participant at least once every two weeks to assess progress to achieving goals for the Individual Service Plan (ISP). Connect participants to services that address immediate needs related to housing, substance abuse, physical and mental well-being, violent or risky behavior, and other areas that threaten to inhibit full participation and ability to achieve personal and program goals. Facilitate Fatherhood groups to improve parenting skills. Facilitate weekly groups with participating fathers.
Responsibilities
Guided by DYCD’s Case Management Toolkit, develop goal-oriented Individual Service Plan (ISP) and facilitate applications and referrals to various services and supports. Address the changing needs of fathers and adapt services accordingly
Facilitate groups utilizing the AM/PM Fatherhood Curriculum
Provide individual, family and group counseling, provide direct services/case planning to clients including mediation/conflict resolution training to fathers to facilitate effective co-parenting and visitation. When necessary, Case Managers will assist with supervised or monitored visitation opportunities for fathers.
Utilize the casework framework principles to provide quality services to individuals, families, and groups. (Engagement, Assessment, Change, & Safety).
Assess father’s immediate needs and current child support order and visitation/placement arrangements.
Maintain contact with program participants at least once every two weeks and follow-up contact for program alumni. (Active caseload of 20-24)
Build effective internal and external working relationships with key stakeholders
Enter progress notes in DYCD data system of record
Qualifications
Bachelor’s Degree.
At least 3 years of experience in human services/child welfare.
E xperience in accessing community resources and working with City systems.
Good verbal and written communication skills.
Proficient in computer skills.
Fluent in speaking and writing in Spanish/English.
Hybrid schedule
(35) an hour work week
