Nurse Case Manager- Manhattan, Brooklyn, Queens & Bronx

Some duties include:

  • Coordinate the delivery of health care services for adults with disabilities residing in residential programs.

  • Be responsible for performing nursing assessments and diagnoses

  • Work in conjunction with other members of the interdisciplinary team to achieve positive health outcomes.

  • Provides guidance and training to direct care staff to ensure proper delivery of health care services.

Requirements:

  • NYS RN License

  • Prior nursing experience and/or experience working with persons with developmental disabilities preferred.

  • Excellent oral and written communication skills.

  • Ability to travel to and access multiple locations throughout the metropolitan area.

  • Able to work flexible hours and/or days.

Registered Nurse- Staten Island & Queens (7:00 AM-3:00 PM/3:00 PM-11:00 PM/11:00 PM-7:00 AM)

Some duties include:

  • Assesses each person receiving services to determine and identify needs, issues, resources, and health care outcomes. Identifies individual’s development strengths and health care needs. Obtains emergency medical care when necessary.

  • Develops and updates health care plan to address assessed needs. Provides direct medical/nursing services to assigned individuals as presented.

  • Oversees the implementation of the health care plan by training persons receiving services, family, and/or caregivers. Trains and oversees staff in carrying out treatment plans relating to health needs on an ongoing basis.

  • Evaluates effectiveness of plan in meeting outcomes. Communicates outcomes to patients, families, caregivers, or service providers.

  • Refers to other health providers, as needed. Schedules follow-up medical appointments according to medical recommendations.

  • Ensures proper documentation of persons receiving services care and progress. Details acute and chronic health problems and interventions; reviews, medication, hospitalizations, clinics, weights, and dietary concerns.

  • Identifies the need for, and arranges and conducts conferences to facilitate resolution of complex issues. Consults with Charge Nurse and/or Nursing Supervisor, as needed.

Requirements:

  • NYS Licensed Registered Nurse (BSN minimum).

  • Minimum 2 years experience working with persons with disabilities.

  • Extensive knowledge of pertinent health resources.

  • CPR Certification.

Nursing Supervisor- Bronx

We are currently seeking a hands-on Nursing Supervisor to provide supervision, training, guidance and evaluations of nurse case managers and both day program and community nurses while working with community living services staff to assure necessary supports are in place for the delivery of health care services to persons receiving services.

Additional responsibilities include:

  • Oversees the nurse case manager’s/Day Program nurses in delegation of nursing tasks to direct care staff and the required associated responsibilities of the nurse, i.e., training.

  • Regularly visits all Residential/Day Program sites within the borough to make certain there is appropriate nursing documentation and follow through. Ascertains all OPWDD regulatory standards regarding medical care needs are being met.

  • Conducts various trainings including orientation, AMAP, and those geared towards nursing and staff development issues.

  • Performs all duties of a nurse case manager for a small caseload and/or when covering in the absence of a nurse case manager.

  • Oversees dissemination of information to concerned parties, including information about acute illnesses, injuries, ER visits, hospitalizations, abuse, elopements, neglect, complex medical problems and a need for informed consent or surrogate decision making.

Requirements include:

  • BSN NYS Registered Nurse with five years nursing experience.

  • Supervisory and/or people with developmental disabilities experience preferred.

  • Excellent oral and written communication skills.

  • Ability to travel to and access multiple locations throughout the metropolitan area.

  • Able to work flexible hours and/or days.
