Description
Comunilife is looking for an experienced Senior Social Worker to fill its open position for their Safe Haven I Program in the Bronx . The Senior Social Worker will conduct psychiatric and other assessments, as well as develop and implement person-centered treatment plans, and supportive services. Services are delivered through evidence-based client treatment practices. Essential Duties
Assess clients’ substance abuse, health, mental health, and other care needs; recommend internal and external referrals;
Provide individual and group counseling;
Prepare psycho-socials;
Facilitate psycho-education groups in relapse prevention, health literacy, and navigating mental health and substance abuse systems, etc. for residents their family and/or support networks;
Provide Comprehensive Case Management services;
Assist clients in improving and maintaining self-care, interpersonal, and vocational skills
Assist clients in achieving housing stability
Manage discharge planning; engage and support clients 90 days post discharge;
Manage client documentation, provide program narrative and/or statistical reports as needed;
Participate in ongoing QI and program evaluation activities
Qualifications
Master’s degree in Social Work (MSW) required; New York State Licensed Masters in Social Work preferred;
Two (2) years’ experience with homeless populations;
Two (2) years’ experience with persons with chronic health and/or mental health conditions and/or substance abuse disorders;
Bilingual/Spanish skills a plus
