RiseBoro Homecare Inc. Care Specialist - Weekend On-Call (Part Time) in BROOKLYN, New York

The Care Specialist -Weekend On Call will be responsible for organizing schedules for our Caregivers to ensure adequate client care. The Care Specialist will also be responsible for providing support and ensuring coverage for client cases and ensuring the aides remain compliant with DOH standards in collaboration with the HR Department. The Care Specialist is responsible for the following:

  • Produce daily reports including but not limited to the SANTRAX/SAM Exception daily report and addressing concerns raised with Home Health Aides, i.e. follow-up calls, merging calls, etc.

  • Review open scheduling orders in the available system and scheduling home health aides to permanent cases/shifts

  • Ensure client services are scheduled as prescribed on authorization documentation

  • Clear exceptions in the system for Saturdays and Sundays

  • Preparing Change Forms to alert the contracts regarding any changes in the client’s service hours, interruptions in services hours, hospitalizations, etc.

  • Understand patient-specific care needs to identify appropriate caregivers to assign to cases

  • Communicate trends, patterns and concerns to the appropriate teams i.e. Coordination, Nursing, Case Management, etc.

  • Responsible for the comprehensive and timely documentation of all interactions in the required databases utilized by Home Care

  • Participate in various quality improvement activities, including contacting clients for feedback related to the quality of services

  • Attend agency meetings, i.e. case conferences, supervision, etc. as well as staff development trainings

    COMPETENCIES

  • 2-5 years of experience in homecare or related field

  • Bilingual (English and Spanish) required

  • Problem solving abilities while multi-tasking; ability to exercise sound judgment

  • Excellent verbal and written communication skills

  • Excellent customer service and organizational skills

  • Knowledge of home care concepts, practices and procedures within a LHCSA

  • Ability to maintain confidentiality at all times

  • Commitment to organization’s mission, vision and values

  • Moderate to strong computer proficiency using standard office software programs required, in particular Microsoft Office; ability to utilize technology in daily work; experience with web-based applications

    EDUCATION EXPERIENCE

  • HS Diploma or equivalent required; Bachelors preferred

  • Computer proficiency - Knowledge of Microsoft office, Excel and PowerPoint

  • Knowledge of SANDATA/SAMS, HHA Exchange or similar home care platforms
