Qualifications Required:

The Pre-Exposure Prophylaxis (PrEP) Patient Navigator/HIV Health Educator is responsible for managing resources through interdisciplinary collaboration to promote health and achieve optimal patient outcomes. The Patient Navigator/HIV Educator will assess readiness for and provide treatment adherence to PrEP medication and retention on PrEP. Coordinate HIV and STI screening, assist as part of a team to deliver HIV test results to patients, conduct patient follow-up and support.

Outreach:You will be responsible for internal/external outreach to community health fairs, large-scale community events, testing events, shelters, food pantries, churches, venue-based outreach, street outreach and outreach to other Community Based Organizations. The position will be responsible for developing relationships with service providers through presentations, tabling, and assisting with the development of linkages.

RESPONSIBILITIESANDDUTIES:

  • Provide education, care coordination, support and services to HIV negative individuals who require assistance in accessing biomedical prevention services.

  • Complete confidential assessments to determine the prevention needs of patients, regardless of HIV status.

  • Provide access to and link patients to biomedical prevention services (PrEP/PEP), STI screening, follow up, including supportive services that can enhance risk reduction, behavior change, adoption, and maintenance.

  • Participate in and conduct community outreach, and enrollment to our Medical Services.

  • Provide referral and assistance in accessing supportive services, health insurance and medication.

  • Maintain patient and program records in accordance with applicable standards, regulations, and requirements. Provide Telehealth Services.

  • Case conference with medical providers and care team

  • Adhere to organizational policy, procedures, and the professional code of ethics.

  • Perform other duties as assigned.

Requirements

Educational Requirements:

  • Bachelor ’s Degree in Social Work, Human Services, Psychology, or related field.

Required Experience:·

  • Experience providing HIV counseling and testing, PrEP/PEP services, case management, care coordination, and community outreach.

  • Health care experience·

  • Knowledge of community resources and linkages, counseling/social work practices.

  • Experience working with high-risk and underserved populations.

  • Ability to assess needs of clients and develop comprehensive care plan.

  • Ability to work independently and as part of a team.

  • Excellent interpersonal, written, and verbal communication skills

  • Ability to use databases to enter data and generate reports.

Technical Skills:MS Word, MS Excel & PowerPoint; and Experience navigating Electronic Health Records (EHR).

Language Skills:Ability to speak Spanish, French or Haitian Creole is a plus.
