Centers Plan for Healthy Living's goal is to create the ultimate healthcare experience that provides our members, their families, healthcare decision makers, and general caregivers with the guidance and plans they need for healthy living.

We are expanding our foot print and seeking compassionate Physical Therapist to provide home-based physical therapy services. Our Therapist contribute to improving mobility and enhancing the quality of life to our members.

Physical Therapist Responsibilities:

  • Travel to and from patients' homes, providing physical therapy as ordered by a physician in accordance with the designated plan of care.

  • Design treatment plans that achieve results through personalized, 1-on-1 individualized care.

  • Evaluate effects of treatment at various stages and adjust treatments to achieve maximum benefit.

  • Consult with patients to learn about their physical condition and symptoms and review their medical history and referrals from doctors or surgeons

  • Diagnose movement dysfunction by listening to patients and observing them as they move about

  • Develop quality, personalized, evidence-based care intervention programs that improve outcomes

  • Use hands-on therapy such as massage to ease patients’ pain and provide stimulation to promote healing

  • Maintain patient records by keeping track of goals and progress while adapting treatment plans accordingly

  • Instructs family members, Home Health Aides, and other caregivers in appropriate physical therapy techniques and what to expect from the recovery process

    Qualifications:

  • Current Physical Therapy Licensure in NY State

  • Graduate of an accredited PT program

  • Proven work experience as a physical therapist

  • Intensive education and clinical expertise

  • Ability to manage patients with different types of personalities

  • Current knowledge of treatment practices

  • Excellent interpersonal communication skills
