Candidates must have the required qualifications to be considered for the position. The salary or hourly rate offer will be based on the candidate’s experience in the exact or comparable position and additional preferred qualifications will be considered.

PURPOSE OF POSITION:

The Intake Specialist will conduct in-depth interviews/intakes with callers that are interested in receiving Case Management, Meals on Wheels, Home Care, and/or Housekeeping Services funded by the Department for the Aging under the supervision of the Case Management Supervisor/Intake Coordinator. Specific duties and responsibilities include:

ROLES, RESPONSIBILITIES AND ESSENTIAL DUTIES:

  • Complete Intake information in DFTA’s client tracking system within DFTA timeframes

  • Screen potential clients for services using a strength-based approach

  • Maintain accurate written records of phone calls related to prospective intakes

  • Refer to appropriate agencies in the community that might be able to assist the prospective client if they are not eligible for DFTA funded services

  • Keep records of all contact with clients, including case files, and unit entries in the computerized client database

  • Initiate referrals to and coordinate activities with the homebound meal and homecare agencies

  • Attend training as required including mandatory DFTA training

  • Perform related duties as required by the Case Management Supervisor and/or Program Director

    REQUIRED SKILLS/QUALIFICATIONS:

  • Two (2) years of experience in providing EISEP case management or social services to older adults is preferred

  • Familiarity with entitlements, benefits, and procedures for applications; experience providing these services for elderly clients preferred

  • Ability to work remotely if necessary

  • Excellent communication skills (oral and written)

  • Excellent customer service skills

  • Ability to understand the concepts of institutional and structural racism and bias and their impact on underserved and underrepresented communities

  • Demonstrated commitment to supporting communities that have experienced systemic oppression and bias (i.e. people of color, LGBTQ+ people, immigrants, justice-involved persons, etc.)

  • Bilingual: Spanish & English required

    SKILLS, KNOWLEDGE & ABILITIES:

  • Bachelor’s degree required; Master of Social Work (MSW) preferred
