Salary: $37,598 to $45,819 Annually Salary is based on education and experience

Job Description: Provides crisis prevention and management, monitoring, assessment, and service planning and care coordination for up to 200 adults with mental illness in the Brazos Valley.Routine Case Management involves accessing medical, social, educational, and other appropriate services that will help an individual achieve a quality of life and community participation acceptable to the individual. The Routine Recovery Facilitator will negotiate and facilitate the array of services and supports needed to address the clients personal goals and desired outcomes as identified through the TRR Assessment and Individual Recovery Plan and provide Rehabilitative skills training as needed.The Routine Recovery Facilitator is responsible for ensuring planned services and support are implemented and appropriately documented as well as advocating and ensuring consistent interactions with other agencies. Education: Requires bachelor's degree in behavioral, social science, or humanities field, educational psychology, or early childhood education or intervention. Experience:Requires experience working with individuals with mental illness, must be able to work independently as well as with a team. Good organizational skills are necessary. Hours of Work: Generally, MondayFriday 8am-5pm; adjustments in work hours may be warranted to meet the needs of the program. Crisis on Call work required as agency need dictates. Licensure/Certification: Ability to be credentialed as a Qualified Mental Health Professional.Valid drivers license and are insurable by agency insurance. Continued employment is contingent upon successful completion of Adult Needs and Strengths Assessment (ANSA) certification as well as Illness, Management and Recovery certification. Physical Requirements: Ability to drive a vehicle; ability to communicate effectively. Facility: Brazos County.
