Child & Family Services of NY Case Planner (Foster Care + Adoption) in Buffalo, New York

Description

The Case Planner provides case work services to an assigned caseload of children and families. They assist parents in meeting court ordered requirements to achieve family reunification; facilitates concurrent planning to ensure timely permanence for child if reunification is not possible.

Major Responsibilities/Activities:

  • Manages an assigned caseload of children in the Foster Care + Adoption program and provides casework services to ensure the child’s safety and well-being and support the child’s permanency planning goal

  • Completes home visits with the youth in foster care in their foster home on minimally a monthly basis, but can be more based on the child's level of care associated with their needs

  • Makes monthly contact with all identified parents of the youth in foster care

  • Schedules and facilitates visitation between parents and youth in foster care and siblings of the youth who may be placed in an alternate setting

  • Provides casework services and support to birth parents in meeting court ordered requirements to achieve family reunification; facilitates concurrent planning to ensure timely permanence for child if reunification is not possible

  • Works in collaboration with and maintains on-going communication with local district case manager and other service providers in facilitating child’s permanency plan

  • Manages the transition of children in placement from one setting to another (e.g. birth parents’ home to foster home, foster home to birth parents’ home or adoption home etc.)

  • Participates in Family Court proceedings to facilitate child’s permanency plan

  • Supports foster parent’s skill development in order to best meet the needs of children in their care

  • Maintains progress notes, case record documentation, and required reports in accordance with district requirements and NYS regulations

  • Models and practices sensitivity, fair treatment and acceptance of diversity in all interpersonal interactions

  • Participates and contributes to program Quality Improvement and Utilization review process

  • Maintains positive working relationships with program staff and others within the agency

  • Demonstrates strong interpersonal skills during interactions with internal and external customers

  • Promotes the services of the program/department, as requested

  • Participates in a department on-call rotation

  • All other duties as assigned by Foster Care and Adoption Supervisor or Program Manager

    Minimum Requirements:

  • Bachelor’s Degree in Social Work (BSW) plus two years paid experience in child welfare preferred, or equivalent combination of education and experience OR a Master’s Degree in Social Work (MSW) with experience in child welfare

  • Valid New York State driver’s license and continued automobile insurance

  • Accessibility to reliable transportation during working hours

  • Ability to work as part of a team

Hours: Mostly traditional business hours, but could include occasional nights and weekends

Competitive Salary: $20.50/hour for 37.5 hour work week
