Evergreen Health Director of Supportive Services in Buffalo, New York

The Director of Supportive Services is responsible for the development, coordination, and implementation of community-based HIV/AIDS support services throughout the Supportive Service grant. This includes medical transportation, housing, food services, emergency financial assistance, health education, case management, and psychosocial support. The position supervises direct service staff including health educators, service coordinators, data staff, and navigation assistant. As part of the essential functions of this role, the Director of Supportive Services:

Essential Functions:

  • Represents the agency in the community through participation in local workgroups and collaborations related to the scope of services

  • Ensures adherence to Ryan White AIDS Institute program standards and develops and implements work plans and goals

  • Contributes to the development and implementation of the Supportive Services program with the Sr. Director of Supportive Services and the AVP of Supportive Services.

  • Supervises staff, maintaining regular supportive communication and assisting with their professional development

  • Coordinates outreach to promote the program with community health and other community based organizations serving the priority population.

  • Develops and monitors the program budgets, work plans, completes monthly reporting, and works with agency fiscal and grants management staff when appropriate.

  • Assists in the maintenance and creation or any procedures as needed

  • Assesses and monitors program compliance with funding requirements including review of notes, conducts chart reviews routinely and work with team to ensure appropriate follow up with patients

  • Organizes and leads events to support outreach and recruitment efforts including flexibility on evenings and weekends when needed

  • Contributes to grant writing activities to support new and existing programming in collaboration with Grants Management and supervisor

  • Orders and distributes any supplies and develops needed social media and multimedia materials in conjunction with the marketing staff

  • Maintains effective communication with grants management, program staff, and quality surrounding monthly and annual reporting, deliverables, outcomes, monitoring, and data

  • Monitors and measures patient satisfaction to determine effectiveness of programs

  • to enhance outreach and program promotion.

  • Assists with the creation of staff development and enrichment programming for the Center of Supportive Services, including planning trainings and guest speakers, as well as coordinating teambuilding activities.

  • Collaborates with all team members to ensure a patient-focused approach to care

  • Ensures the timely entry and accuracy of all program data to promote patient care delivery and participates in ongoing QI/QA activities and training to improve patient experience and increase data quality where needed.

  • Maintains and protects sensitive information in enterprise systems and uses/shares data in compliance with Information Governance and Security Policies.

  • Attends initial orientation, completes required annual trainings, and other trainings to strengthen skills and improve program outcomes .

  • Actively checks work emails, reads all company communications and stays up to date with organizational and department specific announcements.

  • Adheres to policies regarding working hours, break periods and proper use of the ADP payroll and benefits system.

  • Maintains adherence to Evergreen Health Services Corporate Compliance policies; promptly reports any violations to their supervisor, the Corporate Compliance Officer, or anonymously to the Corporate Compliance helpline at: (716) 541-0663.

  • Other duties as requested.

    Qualified Candidate will have BA/BS in Human Services with a minimum of two (2) years of experience working project management services preferred

    Job Type: Full Time

    Required experience:

    Experience developing and implementing direct patient services and case management services preferred. Ability to work a hybrid schedule when there is an approved Alternative Work Agreement in place

    Education and Experience:

    BA/BS in Human Services or related field and two (2) years of demonstrated leadership or project management experience.
