The McGuire Group has an exciting opportunity for a Full-Time Admissions Screener to join our caring and knowledgeable team supporting our facilities in western New York. The admissions screener is works as a liaison between referral sources; and facilities, making both medical and social assessments, coordinating all admissions and transfers to the facility, working closely with the facility admission coordinator and a team of insurance coordinators and leadership in our admission departments.

ABOUT US:The McGuire Group is comprised of 5 skilled nursing and subacute rehab facilities located throughout Western New York which includes five skilled nursing facilities We offer a variety of services including subacute rehab, long-term care, and hospice. We are proud to be part of Living Legends Health including facilities in Absolut Care, Vestra Care and Taconic Health Care.

ABOUT THE POSITION:The successful candidate will perform a variety of duties, including but not limited to:

Responsibilities

  • Responsible for completing admission assessments and recommending patients for long term or subacute care

  • Visit designated hospital assignments daily

  • Knowledge of age specific developmental factors specific to adult and geriatric residents (i.e. physical, cognitive, and socialization factors) in carrying out the admissions process

  • Interacts with residents and family members, co-workers, clinical and ancillary staff in a non-judgmental, supportive and calm manner

  • Additional tasks as assigned

Qualifications

  • Degree in Nursing, Social Work or Physical Therapy preferred

  • Experience in Admissions Screening and long-term care is preferred

  • Good Understanding of Medicare and other Managed Care insurance helpful

  • Must be flexible, professional, and reliable

  • Computer skills including keyboarding and PCC

  • Strong customer service skills

  • Must possess a valid New York State Driver’s License and have transportation available for daily job-related travel

EMPLOYMENT TYPE:FULL-TIME

Pay: $28.00 - $32.00 per hour

Expected hours: 40 per week
