Providence Social Worker MSW - Palliative Care in Burbank, California

Description

Bilingual in Spanish/English strongly preferred.

This position will be traveling to see patients in the Los Angeles/San Fernando Valley area.

The Palliative Care Social Worker participates in the activities of the interdisciplinary Palliative Care Team. The PC MSW participates in the development and review of appropriate protocols, facilitates formal and informal meetings regarding patient and family communication and decision making processes that support appropriate referral and consistent care, works in collaboration with physicians, hospital, home health and hospice health care teams to provide Palliative Care services in patient, out-patient and in home settings to seriously ill patients and their families. The PC MSW engages in independent decision-making about psychosocial needs, provides psychosocial services to individuals and their families, and provides formal and informal meetings regarding patient and family communication and decision making.

Required Qualifications:

  • Master's Degree in Social Work

  • 3 years Inpatient, Outpatient, or home-based Social work

  • Demonstrated evidence of experience in palliative/hospice care settings, ability to:

  • Work independently as well as collaboratively on a small interdisciplinary team,

  • Provide and model interdisciplinary palliative assessment and management of physical, psychosocial and spiritual symptoms during aggressive treatment and end of life care.

Preferred Qualifications:

  • Upon hire: Social Worker License

  • Working knowledge of research techniques, data collection, and policy, program and curriculum development

  • Experience with quality improvement methodology
