Cambridge Health Alliance (CHA) , a well-respected, nationally recognized and award-winning public healthcare system is seeking a full time Clinical Specialist II, Nurse Practitioner, to join our CHA PACE Program . CHA PACE is a Program of All Inclusive Care for the Elderly (PACE) at Cambridge Health Alliance (CHA). CHA PACE serves adults 55 years and older with chronic medical and/or mental health difficulties with the goals of achieving the highest level of community independence and preventing nursing home placement. CHA PACE’s high functioning interdisciplinary team works to fully integrate and coordinate medical and behavioral health care, community resources, and in-home support services.

Perform a variety of duties designed to meet the health care needs of individuals, families, and the community in general. Essential job functions are focused on customer satisfaction, effective use of resources, teamwork, innovation and continuous quality improvement.   • 36 Hours/week, M-F, MNA Cambridge Union

  • Assess the physical and psychosocial health of individuals and families by obtaining health, medical and social histories using the problem oriented (SOAP) recording system.

  • Evaluate, interpret and record the developmental status of the patient and family in order to plan and execute proper intervention.

  • Perform physical examinations. Diagnose health and development problems.

  • Care for patients suffering from acute and chronic diseases by managing consults appropriately with collaborating physician in accordance with guidelines.

  • Initiates, evaluates and modifies the effectiveness of therapeutic plans, collaborating appropriately with other members of the health care team.

  • Orders laboratory and radiographic studies as necessary. Evaluate results and determines which findings require consultation or referrals to other Providers.

  • Work with the community to assess health needs. Assist in the development and implementation of programs designed to meet community needs.

  • Attend conferences and meetings with other agencies, providers, neighborhood health associations and other advisory groups to share information, discuss common problems, give reports and seek consumer input etc.

  • Maintain stock supplies of pharmaceuticals, medical supplies and equipment.

  • Maintain accurate records, and maintain referral books and tickler files for follow-up of specific problems.

  • Participate in quality care of evaluations to include record audits and initiation/revision of guidelines, policies and procedures. Attends and contributes to clinical meeting of specialty and other meetings.

  • Maintain current awareness of new concepts, methods, techniques, regulations, etc., related to the Nurse Practitioner role.

  • Communicate effectively and work as part of a team. Displays ability to get along with co-workers, other departments and the public.

  • Perform other related or similar duties as directed or as responsibilities necessitate.

  • Maintain current Basic Life Support “C” Certification

Ideal Candidates will have a minimum of 2 years of experience. Must be a graduate of an accredited Nurse practitioner program. Current Massachusetts licensure is required authorizing practice in an expanded role as well as current DEA number and Mass DPH registration to prescribe controlled substances.
