CommQuest Services is one of the largest non-profit mental health, addition recovery, and social services organizations in Northeast Ohio, working with clients from more than 34 counties in 12 facilities through more than 20 programs. We help people make positive changes in their lives through in-patient, outpatient and school-based programs, along with supportive services -- from basic needs to employment assistance.

 

 

Overview of Position

 

The Case Manager provides leadership and direction for the complex residents and has the responsibility of maintaining uniformly high-quality service to ensure maximum benefits to residents. The individual must possess strong organizational skills, a broad knowledge of community recourses offered, the ability to work independently, and possess excellent documentation skills. The Case Manager is expected to establish working relationships with diverse individuals and work collaboratively with others on behalf of the client both within the agency and in the community. Other services include ongoing assessment of needs, advocacy and connection to community resources.
