Carlsbad Lifehouse, Inc., a leading provider of mental health services in Carlsbad, New Mexico, seeks a qualified and compassionate Primary Care Provider (PCP) to join our esteemed team. In this vital role, you will provide comprehensive primary care services to a diverse patient population, including members of the general public and residents of our Lifehouse residential and sober living programs. Responsibilities:
- Conduct comprehensive physical examinations and health assessments.
- Diagnose and treat a broad spectrum of acute and chronic medical conditions.
- Develop and implement individualized treatment plans encompassing preventive care, medication management, and referrals to specialists as needed.
- Educate patients on disease management, healthy lifestyle choices, and preventative measures.
- Collaborate effectively with nurses, other healthcare providers, and Lifehouse residential and sober living staff to ensure coordinated patient care.
- Maintain accurate and detailed electronic medical records (EMR).
- Demonstrate a commitment to staying current on the latest evidence-based practices in primary care medicine.

- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) license, Master of Science in Nursing (MSN) with a specialty in Family Practice (CNP), or Physician Assistant (PA) certification.
- Active and unrestricted license to practice in the state of New Mexico.
- Minimum of 2 years of experience in a primary care setting.
- Strong clinical skills in history taking, physical examination, and diagnosis.
- Exceptional communication, interpersonal, and collaboration skills.
- Patient-centered approach to care with a focus on building rapport and trust.
- Proficiency in EMR systems.
