Meets with households seeking assistance to complete the gathering of demographic information and using the Arizona Self-Sufficiency Matrix and working in partnership with the applicants, determines the households self-sufficiency level.

Works with the applicant to create and implement a case management plan incorporating goals and objectives that address identified barriers to self-sufficiency.

Remains in close contact with applicant to monitor case plan activities and provide support and guidance to the applicant. The case manager and applicant will revise the case management plan as necessary to meet changing needs of the applicant/household. Provide follow up after case plan has been closed.

Provides referrals to community resources providing services identified as essential to overcoming barriers to self-sufficiency. The case manager also works closely with community resources to insure that services provided are adequate, appropriate and provided in a timely manner.

Reviews documentation and verification provided by application to determine eligibility for financial assistance from all available funding sources. Prepares funding authorization documents and informs the applicant of rights and responsibilities with regard to assistance provided.

Maintains a hard copy of applicant demographic information, documentation and verifications needed to determine eligibility for financial assistance and other related materials. Maintains an electronic applicant file in the CAHRA in-house database.

Is available to work with other community organizations to provide presentations, attend meetings, trainings and/or conferences.
