As a Case Manager in our Substance Use Disorder (SUD) Program, you’ll provide life-changing support to those transitioning back into the community after incarceration.   • Salary:$26-27/ hour at 35 hours per week

  • Schedule:2-3 days in the Riverhead or Yaphank correctional facilities from 7:30-3:30PM. Remaining days will be in office from 9:00AM-5:00PM

  • Location: Central Islip, NY

    What You’ll Do:

  • Work directly with individuals who are preparing to re-enter society from Suffolk County correctional facilities, offering crucial support in substance use recovery and transition planning.

  • Collaborate with correctional facility staff and community organizations to develop customized reentry plans that connect clients to post-release services, such as healthcare, housing, and vocational support.

  • Facilitate access to essential services like Medicaid, benefits, and Medication Assisted Treatment (MAT) providers, ensuring smooth transitions and reducing the risk of relapse.

  • Act as a trusted guide for individuals re-entering the community, providing ongoing care management and connecting them with peer support networks.

  • Track client progress, maintain accurate records, and contribute to a data-driven approach to improve outcomes for those in need.

  • Enjoy a flexible work schedule with time split between correctional facilities in Riverhead/Yaphank and our Central Islip office.

    What We Are Looking for?

  • A Bachelor’s Degree in Social Work, Mental Health Counseling, Criminal Justice, or CASAC, with at least two years of experience working with substance use or criminal justice-involved populations.

  • Passion for helping vulnerable individuals overcome challenges and succeed in their recovery journey.

  • Strong communication and collaboration skills, with the ability to work with a wide network of community and healthcare services.

  • Proficiency with digital tools and systems, including Microsoft Office and client databases.

  • A valid NYS driver’s license, personal vehicle, and insurance to help transport clients to appointments when needed.
