As the Team Lead, you will be responsible for overseeing patient care in an assigned community or communities per Federal, State, and Local regulations. The Team Lead is responsible for providing services (Therapy and Wellness) and operational support to the rehabilitation service teams to implement company-wide policies, clinical programs, and quality initiatives to optimize each patient’s functional well-being and satisfaction. As the Team Lead, you may be responsible for advising treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices, and ethical standards.

  • Delivers high-quality rehabilitation and wellness services that are person-directed andthat produce outcomes in line with company goals and objectives

  • Develops and coordinates schedules of all services in the community to optimizeresident participation and therapist productivity

  • Demonstrates solid knowledge and understanding across all disciplines

  • Acts as liaison between company and host community through regular communicationwith Executive Director and Director of Nursing/Wellness

  • Promotes EmpowerMe service offerings and provides in-services to care staff regularly

  • Obtains consent and all necessary intake paperwork from resident or Power of Attorney(POA) before initiation of services, including communicating financial obligations asneeded, and submits documentation to business operations support team

  • Provides a summary of residents currently on therapy caseload to the Executive Directorand Director of Nursing/Wellness every week

  • Participates in the assessments of new admissions to identify any functional issues thatcould be improved/addressed through therapy

  • Actively participates in all required meetings and training

  • Attends daily Stand-Up meetings and reports information internally

  • Participates in the community’s regularly scheduled At-Risk Meeting tounderstand what residents are at risk and may determine therapeuticinterventions that would be beneficial

  • Participates in move-in assessments with the community as directed by ADO

  • Maintains therapy gym in “tour ready” condition

  • Participates in quarterly reviews with ADO

  • Meets with families and patients as needed

  • Uploads information from daily Stand-Up form weekly

  • If the community does not hold Stand Up meetings, check in daily with the team to findout the following: tours scheduled where therapy involvement will be helpful,assessments scheduled where therapy involvement will be beneficial, date of the nextAt-Risk meeting

  • Assists with meeting productivity goals and expectations, as identified by the company

  • Treats patients and their families with respect and dignity; identifies and addressespsychosocial, cultural, ethnic, and religious/spiritual needs of patients and their families

  • Complies with HIPAA (Health Insurance Portability and Accountability) and PHI(Personal Health Information) guidelines and always protects confidentiality

  • Provides input in formulating budget, assists in evaluating department performanceversus budget controls, and takes appropriate action to remain within budget guidelines

  • Communicates professionally and clearly, and maintains a good working relationshipwith physicians, care staff, the administrative team, and interdisciplinary teams, as wellas demonstrates flexibility to ensure patient/facility/customer needs are being met

  • Identifies residents who need therapy services through routine screenings,communication with care staff, and monitoring of electronic medical record(s)

  • Implements clinical programs that are focused on the needs of the residents living in asenior living community

  • Administers and collects patient satisfaction surveys after completion of therapy

  • Communicates updates regularly to the team; monitors completion of required training

  • Participates in interviewing and onboarding new team members, including licensedtherapists and assistants across all disciplines and wellness coordinators.

  • Participates in tours and helps prospective residents and family members understandhow EmpowerMe services will address their specific needs

  • Demonstrates competence in regulatory compliance and provides assistance to AgencyAdminister(s), ADO, and Clinical team(s) to aid in continued compliance with RehabAgency regulations within the Agency of participation

  • Provides leadership in such a manner that represents fundamental servant leadershipprinciples, including effective listening, empathy, awareness, and persuasiveness

  • Delivers truthful and unfiltered information to subordinates, supervisors, and companyowners in a clear and respectful manner

  • Maintains professional expertise through continued education as necessitated bylicensure and as appropriate for the provision of specific treatments, utilizingEmpowerMe’s educational resources when available

  • Ensures that company standards and policies are well-maintained and aligns oneselfwith the company vision for service quality

Team Lead Qualifications, Education, and Experience:

  • Degree in healthcare with at least two years of relevant experience in a senior livingsetting

  • Applicable state licensure(s)

  • Exceptional communication and interpersonal skills are required, along with the heartand passion for working with older adults while providing the best care possible.

  • Proven experience in staffing, leading, developing, and retaining solid organizationalteams.

  • Ability to work well with others and take direction from management, as well as take theinitiative- willing to go above and beyond to ensure the needs of the organization aremet in accordance with the company’s core values

  • Passion for achieving results through self-motivation, initiative, and proactive orientation with a sense of professional curiosity, desire to learn new things, and to find/recommend solutions to problems

  • Strong project management and organizational skills with strong attention to detail

  • Respect for the principles of patient/resident rights and confidentiality

  • Must possess a solid understanding of Wellness offerings, Physical, Occupational, and Speech Therapy for seniors, as well as a general knowledge of laws and restrictions regarding Assisted Living, Independent Living, and Memory Care

  • Must exhibit and promote a high level of professionalism, customer service, curiosity, and friendliness in all interactions with employees, residents, and visitors throughout the organization

Occupational Therapist Qualifications, Education, and Experience:

To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below represent the knowledge, skills, and abilities required.

  • Graduate of a bachelor’s, master’s, or doctorate in Occupational Therapy with one (1) year of experience in rehabilitation in senior care communities or related settings is preferred.

  • Current state licensure, in good standing, as an Occupational Therapist or be eligible for temporary license with supervision based on current state guidelines.

  • Basic Life Support/CPR certification

  • Knowledge of Medicare payment systems and EHR/EMR systems

  • Proven ability to build and maintain a patient caseload, specifically within a Med B setting

  • Demonstrate proficiency in occupational therapy modalities and ADLs

  • Exceptional communication and interpersonal skills along with the heart and passion for working with older adults while providing the best care possible

  • Passion for achieving results through self-motivation, initiative, and proactive orientation with a sense of professional curiosity, desire to learn new things, and to find/recommend solutions to problems

  • Understanding of professional curiosity, willingness to learn new things, and to see/recommend solutions to problems

  • Driven to achieve and exceed goals and can motivate others to do the same

  • Professionally conduct oneself at all times with the ability to effectively perform conflict intervention and resolution
