RN/ LPN CASE MANAGER

Position Summary

Provides professional nursing care visits to patients.  Independently provides and documents patient visit.  Assist with in-service and training as assigned. 

Key Responsibilities and Duties

  1. Supervises and is responsible for PCA'S, HHA's, HMK's, ILA's in the field and provides appropriate documentation.
  2. Provides home care nursing visits as assigned and comprehensive client assessments in a timely manner.
  3. Participates in the quality improvement and utilization review activities of the organization along with any data correction procedures.
  4. Submits completed documentation of the home card visit within the established time parameters
  5. Attends multidisciplinary patient care conferences as requested.
  6. Attends staff meeting and in-services
  7. Instructs patients and caregivers according to the plan of care.
  8. Admits patients, develops and updates patient plan of care adhering to the STANDARD criteria for home care provision.
  9. Makes timely nursing assessments and reports significant changes in the patient's condition to the appropriate team members.
  10. Communicates to physician any changes in the patient's condition.
  11. Demonstrates a high degree of commitment to customer service and quality care provision.
