TEKsystems Admissions Director in Cockeysville, Maryland

Description

Responsible for the successful pre-admission and admission experience to include the transition from hospital/acute care to sub-acute rehabilitation; coordination of skilled rehabilitation services, and oversight of per-diem admissions.

  1. Ability to establish rapport with hospital case managers as well as orthopedic, cardiac and neuro physician groups by frequently visiting referring hospitals and providing presentations.

  2. Able to respond to referrals with a prompt one-hour turnaround time.

  3. Organizational skills.

  4. Able to analyze lab values, disease process, and discharge planning goals.

  5. Work closely with various interdisciplinary team members

  6. Ability to access potential patients to be admitted to sub-acute rehabilitation.

  7. Hospital visits to verify admission appropriateness

  8. Review Welcome Packet information with Patient and family

  9. Post admission follow up

  10. Daily interaction with the Hospital Liaison

  11. Providing patient admission criteria evaluations.

  12. Interdisciplinary team interaction

  13. Participate in interdisciplinary care plan meetings, as needed.

  14. Secure approval of insurance and financial information prior to admission to include managed care authorization numbers.

  15. Maintaining up-to-date documentation on patient charts as mandated by regulatory bodies.

  16. Securing resources for patients on the health care unit.

  17. Provide in-services as needed to staff.

  18. Serve on facility wide committees as requested.

  19. Be aware of HIPAA security regulations.

  20. Pre-admission and admission criteria.

  21. Nursing department communication and readiness.

  22. Family interactions prior to and after admission

Additional Skills & Qualifications

Experience working with hospitals to admit patients to Long Term Care/nursing or skilled nursing facilities

Experience in the field of aging.

Comprehensive knowledge of the normal aging process.

Comprehensive knowledge of dementia.

General knowledge of the State and Federal laws and regulations concerning the provision of health care to patients in skilled nursing facilities and assisted living.

Bachelors degree required in relatable field

Marketing or clinical experience preferred.

Has latitude to deal with most problems.

Able to manage utilization review to include managing discharge dates to foresee available admission options.

Pay and Benefits

The pay range for this position is $35.00 - $40.00
