As the Care Coordinator for Maternity and/or NICU, you will have a panel of members identified as having a Low-Risk pregnancy and/or NICU babies. You will provide support and address barriers to care and social determinants of health with the goal to improve health outcomes.

To promote healthy pregnancy, delivery, and postpartum outcomes for both mother and baby, it is essential to provide evidence-based, trimester-specific, and postpartum education. This includes assistance in developing a birth plan, accessing value-added benefits, and understanding covered services, including Medicaid postpartum coverage. Additionally, support should be given for accessing contraceptive care based on family planning needs, conducting mental health and substance use disorder (SUD) screenings with referrals to treatment when necessary, and coordinating with obstetric and other providers for appointment assistance, communication, and advocacy. You will work closely with a team of nurses who manage High-Risk members and who will support you on any clinical decision needs.

This is a Field-Based Role with a Home-Based Office.

If you reside within the Southeast, KS region you will have the flexibility to work in both the field as well as telecommute* as you take on some tough challenges.

Primary Responsibilities:

  • Provide evidenced based trimester specific and postpartum education to promote a healthy pregnancy, delivery, and postpartum outcomes for mother and baby

  • Create a positive experience and relationship with the member and your team

  • Proactivity engages the member to manage their own health and healthcare

  • Support SDOH needs

  • Perform outreach to providers, hospitals, Members, and clinics

  • Attending community events

  • Able and willing to travel within the assigned region per the business needs

  • Performs all other duties assigned

Required Qualifications:

  • Must have one of the following:

  • Bachelor’s Degree in health, human, social work, or education services with 1+ years of qualifying experience in Care Coordination of individuals with complex health conditions, including Care Coordination of Behavioral Health conditions or maternal and child health care experience

  • Licensed practical nurse (LPN) in Kansas with 2 years of qualifying experience working within the maternal and child health population

  • 1+ years of experience with MS Office including Word, Excel, and Outlook

  • Located in the state of Kansas

  • Ability to travel up to 50% of time in assigned region to visit Medicaid members in their homes and / or other settings, including community centers, hospitals, or providers' offices

  • Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)

  • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service

Preferred Qualifications:

  • Knowledge of the culture and values of community

  • Case Management experience

  • Background in Managed Care
