Heritage Ministries is in growth mode and currently seeking a MDS Coordinator (RN) to join our team! The MDS Coordinator (RN) is responsible for the coordination and completion of necessary functions to assure Medicare and other payer’s requirements are maintained in regards to clinical assessment and documentation. The MDS Coordinator will act as a coordinator of MDS, Medicare, and Restorative Nursing functions. This individual will work in conjunction with the Management Company’s Clinical Reimbursement Case Manager, acting as a liaison to finance/billing, and assisting as defined within in Nursing Department functions. Responsibilities:

  • Is responsible to maintain appropriate Medicare Management via regular quality assurance to assess appropriate setting of ARD’s, assure therapy and clinical service capture, audit for direct care documentation of ADL’s, and clinical/nursing documentation of daily skilled needs.

  • Is responsible to track benefit periods and skilled needs as well as establish spells of illness and maintain communication with Clinical Reimbursement Case Manager/ Billing Department regarding benefits.

  • Coordinates Sub-Acute Rehab/ Medicare A meetings, acts as moderator, and provides on-going input regarding resident’s coverage.

  • Will assist the Nursing Department in Case Mix leadership initiatives and will assist in identifying appropriate opportunities to report resident acuities via the MDS and assure that all scheduling and completion adhere to strict timing guidelines.

    Qualifications:

  • Licensed as a Registered Nurse in good standing in Pennsylvania. Maintenance of licensure requirements required.

  • BSN Preferred

  • Long term care experience preferred

  • Knowledge of MDS and Medicare regulations
