Bassett Healthcare Care Manager Registered Nurse in Cooperstown, New York

What you'll do Working under the supervision of the Nurse Manager of Care Management, the Registered Nurse Care Manager serves in an expanded nursing role collaborating with patients and their primary care providers. This individual is a key member of the care team and ensures patients receive quality, efficient, and costeffective healthcare services. The Nurse Care Manager assesses, plans, implements, coordinates, monitors, and evaluates all options and services with the goal of enhancing the patient’s healthcare status. This work will include coordinated efforts involving acute, subacute, rehab, specialty, ED, community and other care areas as needed by developing and maintaining an individualized, interdisciplinary plan of care in collaboration with the patient, family and health care team. The RN Care Manager will utilize a team approach in providing care to the Bassett Healthcare Network patient population and all of its services. Care Management duties performed will include, but are not limited to, assisting the team in identifying high risk patients for hospitalization, preventive/chronic disease care needs, follow up calls, transitions of care, handle appropriate messages from patients, and acting as a liaison and primary conduit for the transmission of information between practitioners and patients. Will also provide advocacy, information and referral services to patients and families to address their medical and psychosocial needs. Provides support and leadership to staff RN’s and LPN’s Provides a positive example for others to follow by demonstrating behavior consistent with a high degree of integrity, teamwork, and collaboration in day-to-day operational activities, and adherence to all policies and procedures 100% of the time with 1-2 exceptions per year as measured by staff feedback and direct supervisory observation Assures all commitments are honored, responds to requests within a defined timeframe, offers constructive feedback to staff and colleagues as measured by staff feedback and supervisory review Supports staff within the department, Establishes timely and effective communications to keep staff aware of important information by communicating with staff and sharing other information in a timely manner as measured by staff feedback and direct observation Is visible and approachable. Elicits concerns, encourage suggestions, and solutions to problems. Communicates in a timely and effective way with others in the department/organization that have the ability to rectify/address issues as measured by staff feedback and direct observation Assists in providing coverage for caseloads as needed Works closely with the Clinic Manager and Nursing Supervisor Sets an example in work performance and team building, concern for patients and co-workers by maintaining a positive interpersonal approach and demeanor, as observed by the manager Accountable for knowledge, understanding, and compliance of operating policies and procedures Works with PC team to ensure that evidence based models are supported and that government regulatory standards are met and maintained to provide excellence in patient care and financial reimbursement Works collaboratively with the Primary Care Nursing team to ensure that staff is current and up to date with all pertinent nursing education and standards relevant to primary care and Care Management Clinical and Technical Responsibilities: Provides Care Management services as appropriate throughout the life span at the Bassett Healthcare Network Health Center assigned In collaboration with the healthcare team develops a plan of care for complex discharges that describes interventions to attain expected patient outcomes 100% of the time as evidenced by supervisory observation, healthcare team and Peer Evaluation feedback Works closely with report writers to analyze clinical outcome data Make revisions in care plan templates, pathways and care management processes based on report results to improve patient outcomes and patient satisfaction. Using accepted criteria, performs initial concurrent and retrospective reviews on 80 - 85% of case load as evidenced by supervisory observation Accurately analyzes patient information to identify correct patient level of care 90 - 95% of the time as indicated by Peer Review Will maintain necessary skills to maneuver through the EHR and all applicable computer systems as evidenced by supervisory observation. Responsibilities for Relationships, Collaboration and Teamwork: Collaborates with other health care team members in providing patient-centered care Acts as care management facilitator to patient care team as evidenced by supervisory observation, healthcare team and Peer Evaluation Works cooperatively and as a resource with internal and external customers using effective communication techniques as evidenced by satisfaction of services through complements from patients, family, or staff Participates in care management activities and institutional committees as appropriate to practice and setting as evidenced by the management of at least 1 care management project/initiative and overall support and participation in departmental initiatives as observed by supervisor Participates in own personal performance appraisal process evidenced by setting appropriate goals and working toward accomplishing them, timely and responsible completion of all pre-appraisal paperwork and arrangements Responsibility for Information: Offers and receives feedback on own performance and the quality of patient care and services delivered Regularly uses established communication systems to stay current with departmental and organizational information and changes as evidenced by promptly reading distributed minutes, organizational publications, and responding to phone calls and emails within 24hrs of receipt Attends 85 - 95% of all staff meetings, assigned committee and organizational meetings and reports back with assignments in 100% of all committee and organizational activities to case management staff Performs other duties assigned as requested and observed by supervisor or manager What we need from you Education: Bachelor's Degree in Nursing or healthcare related field, required Applicants enrolled in a bachelors program with graduation within one year of hire may be considered Experience: Minimum two years nursing experience, required Care/Case management experience, preferred Leadership role experience, preferred Licensure/Certifications: Current NYS licensure as a Registered Professional Nurse, required Valid New York State Drivers license, required BLS certification, required Skills: Community Relations Interacting with People Time Management Computer Skills Physical Requirements: Light work - exerting up to 20 lbs of force frequently, and/or a negligible amount of force constantly to move objects Constant typing/clerical/dexterity, hearing, seeing/monitor/computer use, talking, visual acuity (color, depth, perception) Frequent walking Hazards: Occasional bodily fluids/bloodborne exposure Working Conditions: Occasional dust Travel: Occasional Travel - The job may require travel from time- to-time, but not on a regular basis Benefits: Our commitment to our employees includes benefit programs that are designed to meet the various needs across our employee population. Pay Range Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum: USD $84,409.51/Yr. Pay Range Maximum: USD $126,614.27/Yr.
