AIM Independent Living Center CDPAS Facilitator- Remote Position in Corning, New York

Position Snapshot: The CDPAP Facilitator maintains a minimum caseload of 200 participants who are individuals with disabilities and their caregivers to successfully meet the requirements associated with CDPAP. This can include remaining independent in the community by arranging and administering consumer-directed care.
Essential Functions:
Customer Relations:
Provide new and existing consumers with transitional services.
Ensure time and attendance compliance by monitoring usage and providing education where needed.
Return all consumer calls within 24 hours, and promptly address all questions and concerns as needed.
Staff Consumer Assistance & Education:
Provide ongoing assistance with aide recruitment practices as needed.
Provide ongoing education and best practices for managing their own care & personal assistants.
Upon request by the consumer, provide support with program activities such as hiring, firing, and conflict resolution with employees
Organization:
Document all encounters with consumers, caregivers, and informal supports
Ensure proper review and submission of all required documentation for the timely enrollment completion.
Education and Experience:
Associate’s degree in human services with at least 1 year of related experience, or
Any combination of education and experience that provides the knowledge, skills, and ability necessary to perform the essential functions of this position to the satisfaction of AIM’s ED and,
Experience advocating and facilitating IL services for persons with disabilities, preferred and,
Strong customer service skills preferred.
Qualifications:
Strong interpersonal, communication, organization, and writing skills.
Computer Literacy – including but not limited to: skills in working with MS Office, and MS Outlook, internet research.
The ability to multi-task and work effectively in both a team and individual setting.
Flexibility and ability to manage a complex workload in varied work environments.
Experience with database software.
Valid driver’s license with satisfactory driving record and reliable transportation.
Physical and Environmental Requirements:
Sitting at the computer.
Use of telephone including headset/headphones.
Some travel may be required.
Hybrid and/or fully remote positions are available.
