Overview:Adoration Health is seeking two dedicated Licensed Practical Nurses (LPN) for full-time positions in Covington, TN. This role offers a consistent 36-hour workweek, where you will provide essential care to a patient as part of a specialized care team. Schedule:

• Saturday, Sunday, Monday (7:00 AM - 7:00 PM)

• Full-time: 36 hours per week

Responsibilities:

• Provide high-quality nursing care, including medication administration and health monitoring, in a one-on-one home care setting.

• Maintain accurate documentation of patient conditions and care provided.

• Collaborate with the healthcare team and patient’s family to ensure comprehensive, personalized care.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice

• Provide patient and family teaching according to plan outlined by the RN

• Monitor patient's vital signs and make specific measurements and observations

• Monitor and record intake and output. Report significant changes immediately

• Observe, assess and record patient's physical and mental condition and reports any changes

• Administer medications as instructed and as ordered by the physician and instructed by the RN and document the patient's file accordingly

• Attend and participate in multi-disciplinary patient case conferences, staff meetings and in-service education programs as assigned

Qualifications

• Licensed Practical Nurse / LPN license in good standing

• Previous private duty, home health, hospice, hospital or skilled nursing experience preferred

• Active CPR certification

• Familiarity with electronic health records

• Strong problem solving and critical thinking skills

• Reliable transportation, valid driver's license and current automobile insurance
