Cuyuna Regional Medical Center RN - Clinic Coordinator in Crosby, Minnesota

  • POSITION SUMMARY

  • At the general direction of the Clinic Nurse Manager, the RN Coordinator/Support RN Primary Care is part of an interdisciplinary team that consists of the Clinic clinicians, LPN/CMA's and the RN. This position will support all teams in Family Practice and Internal Medicine. Registered Nurses must continually strive to provide the highest quality care. Duties included but not limited to patient assessment, education, medication and vaccine administration, performing of tests and procedures as ordered, appropriate documentation, and serving as the communication link between the patient, the physician and other clinic departments or agencies involved in the patient's care. Proper documentation of medication refills and telephone encounters with patients. Complies with Minnesota state law and the Nurse Practice Act.

  • POSITION QUALIFICATIONS

  • Education and Experience:

  • Prior clinic nursing experience preferred.

  • License/Certificates:

  • Minnesota RN license. American Heart Associate BLS certification required within 30 days of hire.

    (Baxter RN Coordinator will require American Heart Association ACLS within 4 months of hire).

  • Supports the Nurse Manager in supervision of Nursing Staff and is a role model for nursing by conducting self in a professional manner at all times.

  • Working knowledge of Electronic Health Records (EHR) systems.

  • Demonstrates effective communication and interpersonal skills.

  • The Nurse Coordinator is accountable for own professional development and learning; attends internal and external educational offerings, in-services, and unit meetings. Keeps current in nursing practice standards and promotes an environment of collegiality and collaboration. Serving as mentors, participating on committees and demonstrating flexibility.

  • Remains up to date on new nursing skills and technology, utilizing the nursing process (observation, assessment, planning, implementation, and evaluation) in the delivery of care.

  • Provides leadership and promotes teamwork.

  • Exemplifies self-direction with good organizational, analytical and interpersonal skills.

  • Knowledge and compliance of state and federal accrediting agencies.

  • ESSENTIAL RESPONSIBILITIES

    1. Direct Patient Care

    a. Perform procedures and treatments as ordered such as, but not limited to, ABI, pregnancy testing, EKG, neurometrix, bladder instillation or IV insertion. All duties listed or any other skilled nursing procedure will not be assigned without proof of competency or simulation training is completed by CRMC.

    b. Prepare and administer medications or vaccines according to standard procedure and as ordered by the physician or standing order.

    c. Operate correctly a variety of pieces of medical equipment including, but not limited to, EKG, tympanometer, suction, oxygen, spirometer, audiometer, nebulizer, Dose Response, ABI, neurometrix, wheelchair, and exam tables.

    d. Assist other RN/LPN/CMA staff as needed.

    e. Perform or assist with dressing application and removals as ordered.

    f. Instruct patient appropriately regarding prescribed therapies and preparations for ordered procedures.

    h. Provide patient education and instructions.

    1. Indirect Patient Care

    a. Documents vitals, treatments, procedures, interventions, medications, and essential notes in the patient's medical record according to clinic policy.

    b. Assist physician and reception/scheduling staff in maintaining an accurate daily schedule providing efficient patient flow.

    c. Schedule lab tests, x-rays, consults, surgeries, hospital admission or other appointments as ordered by physician, seeing that all necessary authorizations are complete.

    d. Call, fax, or electronically send prescription refills as ordered by physician or standing order and document according to policy.

    e. Facilitate communication between patient and physician.

    f. Manage voicemail by recording appropriate greetings, checking voicemail, tasking messages to physician for follow up and providing timely follow up to patient.

    g. Attend nursing department meetings.

    h. Demonstrate Standards of Excellence when other duties are assigned.

    1. Prescription Refill Management

    a. Review all refill requests for Family Medicine and Internal Medicine.

    b. Execute medication refill standing order.

    c. Clarify refill requests as needed with pharmacy, physician, mid level, or patient.

    d. Utilize EHR to determine if the patient has received the appropriate labs and office visits as indicated in the standing order.

    e. Document refill or status of refill request electronically according to clinic policy and procedure.

    f. Review patient record in EHR as needed per policy.

    g. Determine status of refill request and reassign as appropriate.

    h. Communicate status of refill request to patient and obtain additional information as needed.

    1. Chronic Disease Management

    a. Utilize patient registries to identify necessary follow up per guidelines for specific disease/conditions.

    b. Assist with audits.

    c. Work collaboratively with physicians to manage patient panel and provide feedback.

    d. Keep abreast of current guidelines related to chronic disease management.

    e. Assist in educating providers and nursing staff.

    The pay range for this role is $33.34-42.46.
