Description

Full Time - 40 hours a week

Salary - $83,000 - $85,000

Sign on Bonus - $5,000.

Bilingual - Spanish/English required

The Universal Home Visitation (UHV) Registered Nurse is responsible for providing comprehensive community health nursing services through homecare for families enrolled in the UHV Program in accordance with program goals, objectives, and protocols. To assess the health needs of infants and their caregivers, assess behavioral health and social needs of families and connect them with appropriate community services.

Essential Functions:

*Contribute to the Cooperative’s commitment to equity and continuous quality improvement that aligns with the agency’s mission, goals, and advancement. * Effectively cross-collaborate with agency staff, leaders, and board members to foster a comprehensive approach of information sharing, working committees, and collective representation. * Routinely disseminate relevant information, presentations, trainings, and learning opportunities for internal program and agency staff. * When applicable, will participate in relevant staff development opportunities (training, summits, conferences, etc.) to increase skill development, elevate program goals, and engage staff members with timely key learnings. * Keep records of client observations and activities as required. Document information for family assessments and program evaluation. Documentation is expected to be done and letters sent out within 48 working hours of the home visit encounter. * Provide a health and physical assessment of mother and/or baby in the home to determine wellbeing. * Assess risk factors for family, including depression, substance abuse, and domestic violence. Exercise clinical skills in appraising situations and making decisions. * Establish a trusting relationship with families during in-home visits and engage the family to establish a family support plan with goals, objectives, and activities that address family strengths and needs. * Assist parents in making appointments for doctors, social service agencies, etc. Act as a liaison between families and other community agencies, serving as family advocate as required. * Work with team to ensure appropriate referrals and connections to community resources as needed. Identify gaps in community services that may be addressed by local partners. * Apply working knowledge of parent-child interaction, child-maternal health, child development, and child abuse/neglect to help families improve parenting skills, increase parenting supports, reduce stress, and address needs of parents; assist in creating a health plan for entire family. * Respond to client needs through flexible scheduling of home visits. Apply effective time-management skills, organization, and multitasking skills. * Provide respectful, culturally appropriate care and education to all clients and their families. * Participate in weekly case conferences, quarterly reviews, and other activities to support delivery of services with quality and fidelity.

Requirements

  • Associate’s degree in nursing

  • Registered Nurse licensed in the State of New Jersey

  • 1 year of relevant nursing experience

  • 2 years Maternal Child Health experience

  • 2 years experience in community health nursing or community outreach

  • 2 years demonstrated cultural competency and experience working with diverse populations

  • Basic computer skills necessary

  • Valid driver’s license, current insurance policy and car

  • Good organizational skills: ability to prioritize work requirements.

  • Ability to communicate well with consumers and providers.

Physical Requirements:

  • Must be able to see to read documents and hear well enough to communicate with others.

  • Must be able to write, type on a computer, and use a telephone.

  • Must be able to operate a motor vehicle.

  • Must be able to lift up to 20 pounds.

Work Environment:

Travels regularly to sites where traffic, weather, and other environmental conditions may be encountered; otherwise works in a climate-controlled professional office environment generally free from hazards and other adverse working conditions.
