UnitedHealth Group Transplant Nurse Case Manager RN - Remote in Dallas, Texas

As a Transplant Case Manager, you will drive positive outcomes by supporting a diverse member population with education, advocacy and the connections to the resources they need.

This role is required to work Monday - Friday from 8:00am to 5:00pm (with some flexibility).

You’ll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges.

Primary Responsibilities:

  • Makes outbound calls to assess members' current health status

  • Case manages members in the pre-operative and post-operative phases

  • Identifies gaps or barriers in treatment plans

  • Provides patient education to assist with self-management

  • Interacts with Medical Directors on challenging cases

  • Coordinates care for members

  • Makes referrals to outside sources

  • Coordinates services as needed (home health, DME, etc.)

  • Educates members on disease processes

  • Encourages members to make healthy lifestyle changes

  • Documents and tracks findings

  • Makes welcome home calls to ensure that discharged member receive the necessary services and resources

Required Qualifications:

  • Current, unrestricted RN license in your state of residence

  • Multiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses

  • 3+ years of experience as a Registered Nurse (RN)

  • 1+ years of Transplant experience working with solid organ or bone marrow as a Nurse

  • Basic level of proficiency with Microsoft Word, with the ability to navigate a Windows environment

  • Access to high-speed internet from home (Broadband cable, DSL or fiber)

  • Dedicated workspace from home

Preferred Qualifications:

  • Certified Case Manager (CCM)

  • 5+ years of Transplant experience working as an RN

  • 3+ years of experience working as a Transplant Coordinator

  • Managed care experience

  • Case Management experience

  • Reside in compact RN license state

California, Colorado, Connecticut, Hawaii, Nevada, New Jersey, New York, Rhode Island, or Washington Residents Only: The salary range for this role is $58,300 to $114,300 annually. Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. UnitedHealth Group complies with all minimum wage laws as applicable.
