Summary:

At [Health Quest Home Care/Western Connecticut Home Care], we are seeking individuals to become part of our exceptional home healthcare team. Our home health team works closely with patients’ physicians and families to provide care in the one place where our patients want to be, in the comfort of their own homes. Responsibilities:

  1. Manages the quality assurance performance improvement program and the Value Based Purchasing (VBP) program metrics for home care to include quality management of the OASIS data & reporting; process & outcome reports; occurrence reports; home health compare & patient satisfaction data; program quality monitoring data; utilization data, payer inquiry requests, denial activity; development of policies & procedures. Uses data trending to initiate & sustain performance improvement activities. Coordinates findings with billing & finance as appropriate.

  2. Serves as a regulatory resource for Federal, State and Joint Commission regulations & standards. Coordinates the regulatory surveys at the agency with the Home Care Administrator and Vice President, Home Care Operations.

  3. Develops, monitors, and evaluates the corrective action plans for deficiencies identified through the quality review process and survey process. Coordinates findings with billing & finance as appropriate.

  4. Oversees the coding and submission of new patient admission, recertification and discharge information.

  5. Evaluates the need for and plans/coordinates special classes, conferences, and meetings to improve level of staff performance and quality of care.

  6. Participates in the weekend/holiday clinical supervisor schedule.

  7. Fulfills all compliance responsibilities related to the position.

  8. Performs other duties as assigned.

Other Information:

Requires a minimum formal education of RN associate degree or bachelor's degree in physical therapy and minimum of three years job-related experience.

License/Credentials: Current CT Registered Nurse (RN) or Physical Therapist (PT) license and Adult BLS Certification

Desired:

Registered Nurse: Bachelor's degree

Physical Therapist: Doctor of Physical Therapy (DPT) degree or a Master of Physical Therapy degree

Working Conditions:

Manual: significant manual skills/motor coord & finger dexterity

Occupational: Some occupational risk

Physical Effort: Medium to Heavy effort. May exert up to 35 lbs. force

Physical Environment: Some exposure to dirt, odors, noise, human waste, etc.
