Case Manager - Residential

This position is responsible for managing an assigned caseload of clients and assessing needs, developing, implementing and reviewing case management plans, and working with other community resources in meeting/achieving client service needs.

This position is Sunday 8am to 6:30pm and Monday through Wednesday 11:00am to 9:30pm.

Essential Job Functions

-Conducts case management assessments as needed -Develops, revises, and implements case management plans in cooperation with participants needs, and maintains proper documentation (must include all needs, appointments and how the participant will get to them: who, what, when, where, why, how) -Detailed documentation; including case management progress notes and an updated case management plan at every contact and change in plan -Maintains caseload and accurate records and submits reports in a timely manner -Maintains monthly contact with caseload -Contact will be made with participants on the day before all appointments to remind them and ensure transportation and/or other obstacles have been accounted for support, especially after periods of hospitalization or incarceration. -Conduct outreach both telephonically and in the community to assist with motivation in treatment, and identify any barriers to successful engagement. Provide education and support on the use and availability of medication assisted treatment. -Works cooperatively with referral sources to co-serve participant needs -Assists participants in all obtaining access to all aspects of care, including job skills, employment, education, housing, child care, intervention services, substance abuse education, mentoring, coaching, and spiritual and faith support etc. -Transportation for participants to and from treatment or recovery support activities -Performs other related work as assigned.

Knowledge, Skills and Abilities

-Introductory knowledge of substance use disorders and their effects on the individual and family members including human behavior, attitudes, and motivations and their applicability in the treatment/rehabilitation of the substance abuser and/or offender. -Ability to effectively manage a caseload, conduct assessments, write assessment summaries, and develop client progress reports.

The case manager must be able to prioritize situations and manage time, and may find that they are needed at irregular hours causing fatigue and stress. The Case Manager may experience stress and stress related symptoms due to interacting with clients in crisis. S/he may be exposed to illness and unsanitary conditions so must take care to protect their own health. They may also have to manage physically threatening clients and be prepared to be responsible for their own safety, and the safety of others.

Work Environment

Work is performed both in an office environment and out in the community.

Qualifications

Minimum of a high school diploma and 2 years of experience in a health or human service agency

OR

Associates degree preferred with major in psychology, social work, and counseling (with a clinical specialty in the human services) or other related field and 1 year of clinical experience in a health or human service agency.

Must possess a valid drivers license and be insurable. This position is subject to a criminal background clearance.

Pay: $17.00 - $19.00 per hour, Full time with complete benefit package.
